Study On The Effects Of Nanofat Grafting Under A Split-thickness Skin Graft Post-operation

Purpose:Full-thickness skin grafts will soon be found to have adverse reactions such as pigmentation and stiffness.Through the treatment of patients with full-thickness skin grafts by nanofat graft,the long-term effects on skin grafts are observed to provide new possibilities for clinical treatment.Method:In the 62 patients,nanofat grafting and full-thickness skin grafting operation were performed for those who met the inclusion criteria,and conventional full-thickness skin grafting operation was performed for those who did not meet the inclusion criteria.Observe and record the surgical effect and postoperative complications of the grafting area immediately after removing the suture.After 6 months,follow-up photos of skin grafts of 62 patients were taken,and the OSAS scores were evaluated and recorded.Result:Immediate situation of suture removal after operation:Of the 62 patients with full-thickness skin grafts,47 cases had good skin survival,9 cases had normal skin survival(with spots and purple),6 cases had poor skin survival(some kind of skin margin infection).Those with poor skin survival can heal within 1 week after 2-3 times of dressing changes;all patients heal clinically without the need for a second operation.Skin pigmentation:Of the 62 patients,12 patients were in the nanofat group,1 patient deepened pigmentation due to infection,and 11 patients in the nanofat group improved pigmentation compared with conventional full-thickness skin grafting.The values of ΔL*,Δa*,Δb*in the two groups did not conform to the normal distribution by K-S test,respectively.The results of Mann Whitney U test showed that the average value of △ L*in the nano fat group was higher than that in the conventional skin grafting group,P<0.05,with statistical significance.The range of L*in the nanofat group was-18.43~-14.15,and that in the conventional skin grafting group was-25.91~-3.46;the average value of Δa*in the nanofat group and the conventional skin grafting group was not significantly different,P>0.05,with no significant difference.The range of a*in the nanofat group was-3.57~-6.10,and-8.24~-7.50 in the conventional skin grafting group;compared with the conventional skin grafting group,the Δb*value in the nanofat group decreased,the range of-7.11~-1.45 and-2.01~-18.35 in the conventional skin grafting group,P<0.05,with statistical significance.OS AS score:The pigmentation score sample does not conform to the normal distribution,Mann-Whitney U test analysis of the nanofat group(5.6667±2.05971),the conventional skin graft group(7.04±1.57739),Z value-2.375,P<0.05 was statistically significant.The softness score samples conformed to the normal distribution.The independent sample t test analyzed the nanofat group(2.75 ± 1.3568)and the conventional skin graft group(5.7±1.54193).The t value was-6.079,P<0.05 was statistically significant.Conclusion:1.Autologous nanofat graft can effectively improve the effect of full-thickness skin graft pigmentation.2.Autologous nanofat graft can effectively improve the texture of the full-thickness graft skin and improve the pliability of the graft skin.
