| ObjectsPreliminary exploration of the syndrome elements,distribution and combination characteristics of Parkinson's disease will provide a basis for the establishment of a Parkinson's disease syndrome standard system.Methods1 The "Responsible Group for the Observation of TCM Syndromes of Parkinson's Disease"was developed by the responsible unit of the research group.After unified training,cases were collected by a number of hospitals in Beijing and Hebei,and a total of 195 patients were included,which is a cross section of clinical epidemiology Interview.2 Use Excel software to establish a database,SPSS20.0 for data statistics.Use factor analysis to explore the common syndrome elements of Parkinson's disease,combined with the clinical experience of clinical experts,determine the number of common factors(symptom elements)and the symptoms of each common factor,establish a function equation,calculate the common factor score,and statistical evidence Distribution characteristics of weather elements,and explore the characteristics of syndrome element combination through cluster analysis.Results1 Factor analysis showed that the KMO statistic of the partial correlation between variables was 0.662>0.5,indicating that the degree of partial correlation between variables was strong,and the effect of factor analysis on the data was better.The approximate chi-square of the Bartlett spheric test is 6992.230(P<0.001).It can be seen that each variable rejects independence,has a strong correlation,and is suitable for factor analysis.2 Factor deficiency,and Wind account for the highest proportions,accounting for 31.8%,28.2%,and 21.0%respectively;followed by Wet,Blood stasis,and Qi stagnation,Fire,Sputum,and Yang deficiency,of which the proportion of wet is high,accounting for 11.79%;the main organs involved are liver,kidney,and the secondary organs are spleen and heart;in the overall distribution of syndrome elements,the proportion of deficiency syndrome is 60.5%,and the empirical proportion is 39.5%.4 Female patients had a higher proportion of Qi deficiency 34(32.4%),Yin deficiency 30(28.6%),Wind 19(18.1%),Wet 14(13.3%);Male patients had Qi deficiency 28(31.1%),Yin deficiency 25(27.8%),Wind 22(24.4%),Wet 9(10.0%)accounted for a higher proportion.Pearson chi-square test P=0.702>0.05,showing that there is no significant difference in the distribution of the syndrome elements of Parkinson's disease in gender in this study.5 Age was positively correlated with blood stasis,with a correlation coeficient of 0.172(P<0.05);age was positively correlated with Qi deficiency,with a correlation coefficient of 0.169(P<0.05);age was negatively correlated with Qi stagnation,with a correlation coefficient of-0.017(P<0.05).6 H&Y classification is positively correlated with wind,Qi deficiency,Qi stagnation,Yin deficiency,blood stasis,and fire.Among them,it is positively correlated with wind,with a correlation coefficient of 0.502(P<0.01);positively correlated with Qi deficiency,with a correlation coefficient of 0.341(P<0.01);Positive correlation with Qi stagnation,with a correlation coefficient of 0.237(P<0.01);positive correlation with Yin deficiency,with a correlation coefficient of 0.188(P<0.01).7 The proportion of Qi deficiency 46(31.1%),Yin deficiency 40(27.0%),Wind 30(20.3%),and Wet 20(13.5%)in the early stages of TCM syndromes is higher;Qi deficiency 16(34.0%),yin deficiency 15(31.9%),and wind 11(23.4%)accounted for a relatively high proportion of TCM syndrome elements in the middle and late stages of the disease.Pearson chi-square test P=0.571>0.05,which shows that there was no significant difference in the distribution of the symptoms of Parkinson's disease in the stage of the disease in this study.8 TCM syndromes in patients with tremor type account for a higher proportion of Yin deficiency 17(29.8%),Qi deficiency 13(22.8%),and Wet 11(19.3%);patients with less exercise-tonicity type Among the elements,Yin deficiency 10(43.5%)and Qi deficiency 9(39.1%)accounted for a higher proportion;in patients with mixed exercise classification,the syndromes of Qi deficiency were 40(34.8%),Wind 35(30.4%),and Yin deficiency 28(24.3%)has a higher proportion.Pearson chi-square test P=0.002<0.01,which shows that there is a significant difference in the distribution of Parkinson's disease syndrome elements in the movement classification in this study,that is,The proportion of the syndrome elements of Qi deficiency,Yin deficiency,and Wind in patients with mixed Parkinson disease is higher,and the syndrome elements of Wet is higher in patients with tremor-type Parkinson's disease.9 The 80 symptom information items were included in the cluster analysis as variables and combined with clinical reality,the clustering results were intercepted into 7 categories,namely liver and kidney deficiency syndrome,qi and yin deficiency syndrome,spleen and kidney qi deficiency syndrome,and wet block syndrome,wind and fire internal action syndrome,qi stagnation and blood stasis syndrome,phlegm and stasis syndrome.Conclusions1 Parkinson's disease mainly involves 9 types of syndrome elements,the main syndrome elements are Qi deficiency,Yin deficiency,and Wind,followed by Wet.Blood stasis,Qi stagnation,Fire,Sputum,and Yang deficiency.The deficiency syndrome is more prominent.The main organs involved are liver and kidney,and the minor organs are spleen and heart.2 Syndrome elements Qi deficiency,Yin deficiency,Wind,and Wet throughout the disease.Qi deficiency,Yin deficiency,and Wind account for a higher proportion of patients with mixed Parkinson's disease,and syndrome elements of Wet has a higher proportion of patients with tremor type Parkinson's disease.3 Symptoms of Parkinson's disease are mixed with deficiency and excessive syndrome,clustering into syndromes:liver and kidney deficiency syndrome,qi and yin deficiency syndrome,spleen and kidney qi deficiency syndrome,wet block syndrome,wind and fire internal action syndrome,qi stagnation and blood stasis syndrome,phlegm and stasis syndrome.The syndrome elements of this disease are both deficiency and excessive syndrome,the syndrome element combinations mainly include liver and kidney,qi deficiency,yin deficiency,wind movement,wet and turbidity. |
