| Objective:Under the guidance of Professor Li Zhidao’s idea of controlling acupuncture sensation, this study,by means of random grouping and control experiment, evaluates the improvement degree of Xingqi method and conventional acupuncture method in the aspects of sleep state at night, daytime state, emotion and Traditional Chinese Medicine syndrome performance in the clinical treatment of insomnia of heart-kidney disharmony type through scientific statistical method on the basis of summarizing the previous clinical work.To observe the clinical effect of Xingqi method in treating insomnia of heart-kidney disharmony type,in order to provide an effective basis for the flexible application of acupuncture technique, the key element in clinical treatment.Methods:72 cases of insomnia of heart-kidney disharmony type were randomly divided into two groups, experimental group and control group,36 cases in each group. The acupuncture points of Baihui,Sishencong, bilateral Neiguan, bilateral Shenmen, bilateral Zusanli, bilateral Sanyinjiao and bilateral Taixi were taken in both groups. In the experimental group, the direction and depth of the acupuncture were strictly controlled, and the acupuncture sensation was conducted to the expected direction along the meridians where the acupoints were located by Xingqi method. If the effect of the transmission of the acupuncture sensation along the meridians was still difficult to achieve after the acupuncture,Xingqi method could be carried out after waiting for the qi to move under the needle. If the direction of the sensory transmission was opposite to the expected direction, the method of cut-off could be used to adjust it to the desired direction. The control group was treated with conventional acupuncture method according to Science of Acupuncture and Moxibustion. Two groups were treated three times a week, six times for a course of treatment,a total of two courses of treatment.By statistical analysis of the scores and changes of each observation index before treatment, after treatment for one course, after treatment for two courses and after treatment for one month follow-up,according to Guiding Principles for Clinical Research of new Chinese Medicineand Pittsburgh Sleep Quality Index(PSQI)score reduction rate, the therapeutic effect of the two groups was determined, and the improvement degree and difference of sleep state at night, daytime state, emotion and Traditional Chinese Medicine syndrome performance of the two groups in each stage were compared.Results:1.Analysis of baseline data:There was no significant difference between the two groups in gender, age, course of disease, occupational impact, combined basic diseases, body mass index and therapeutic effect observation indicators before treatment(P>0.05),indicating that the two groups were comparable.2.Comparison within the clinical efficacy evaluation group:After two courses of treatment, the difference between the two groups of subjects ’efficacy observation indexes was statistically significant(P<0.01),indicating that the two groups of subjects’ efficacy observation indexes after treatment were significantly improved compared with that before treatment.3.Comparison of clinical efficacy evaluation between the two groups:After one course of treatment, two courses of treatment and one month follow-up after the end of treatment, there was statistically significant difference in the total score of PSQI between the two groups(P<0.05),indicating that the overall improvement degree of the experimental group was better than that of the control group.Among them, after one course of treatment and two courses of treatment, the total score of PSQI between the two groups was significantly different(P<0.01).After one course of treatment, two courses of treatment and one month follow-up after the end of treatment, the differences between the two groups in Traditional Chinese Medicine Syndrome Scale scores were statistically significant(P<0.01),indicating that the overall improvement of the experimental group was better than that of the control group. After two courses of treatment, the difference of HAMD score and HAMA score between the two groups was statistically significant(P<0.05),indicating that the overall improvement of the control group was better than the experimental group.4.Comprehensive efficacy judgment:The total effective rate of the control group was86.11%,and that of the experimental group was 97.22%.After two courses of treatment, there was a statistically significant difference in the overall efficacy between the twogroups(P<0.05),indicating that the overall efficacy of the experimental group was better than that of the control group.5.Safety evaluation:There was no significant difference in safety evaluation between the two groups(P>0.05),indicating that the safety of the two acupuncture treatment methods was comparable.Conclusion:1.Acupuncture treatment can significantly improve the nocturnal sleep status, daytime status, anxiety and depression mood and Traditional Chinese Medicine syndrome performance of insomnia patients with heart-kidney disharmony type.2.In improving the clinical symptoms of insomnia patients with heart-kidney disharmony type, the effect of Xingqi method is superior to that of conventional acupuncture method.Xingqi method has advantages over conventional acupuncture method in terms of short-term(within about one month)efficacy, but may be slightly worse in terms of long-term(over one month)efficacy maintenance after the end of treatment.3.In improving the Traditional Chinese Medicine syndrome performance of insomnia patients with heart-kidney disharmony type, the improvement degree of Xingqi method is better than that of conventional acupuncture method.4.In improving the anxiety and depression of insomnia patients with heart-kidney disharmony type, the improvement degree of conventional acupuncture method is better than that of Xingqi method.5.The total effective rate of conventional acupuncture method treatment was 86.11%,and that of Xingqi method treatment was 97.22%.The overall effect of Xingqi method is better than that of conventional acupuncture method.6.In the safety evaluation of the two acupuncture treatment methods, there is no significant difference between Xingqi method and the conventional acupuncture method. |
