| Objective : To investigate the safety and effectiveness of low pressure laryngeal mask airway(LMA)inflation in gynecological laparoscopic surgery,and observe the impact of postoperative complications.Methods : 132 adult female patients with gynecological laparoscopic surgery were randomly divided into observation group and control group,66 cases in each group.The observation group: after laryngeal mask airway was inserted,the pressure in the sacculus was adjusted by using a manometer to make the oropharyngeal leakage pressure(OLP)reach 20 30 cm H2O(1 cm H2O = 0.098 k Pa),and the minimum intracuff pressure(ICP)was recorded.The control group: after laryngeal mask airway was inserted,5 15 ml air was injected to make OLP reach 20 30 cm H2O,and then the pressure in the sacculus was recorded.The same ventilation parameters were given to the patients in the two groups after successful laryngeal mask implantation;the peak airway pressure(Ppeak),the incidence of laryngeal mask airway leakage and the amount of muscle relaxant were recorded during the operation;the recovery time,gastric tube drainage,whether the surface of laryngeal mask was stained with blood when pulling out the laryngeal mask,and the incidence of postoperative complications such as throat discomfort,sore throat and gastric distension were recorded.Results : In the two groups of laparoscopic gynecological surgery,Ppeak was no more than 25 cm H2O,OLP was slightly larger than Ppeak,the observation group: Ppeak was (19.3 ± 4.9)cm H2O,OLP was(20.3 ± 4.1)cm H2O,control group: Ppeak was(18.7 ±3.8)cm H2O,OLP was(21.7 ± 3.9)cm H2O,There was no significant difference in Ppeak and OLP between the two groups(P > 0.05).Compared with the control group,the ICP in the observation group(27.6 ± 6.8)cm H2O was significantly lower than that in the control group(76.8 ± 14.2)cm H2O,the difference was statistically significant(P <0.05);the incidence of postoperative complications such as throat discomfort,sore throat and gastric distension in the observation group was also significantly decreased,the difference was statistically significant(P < 0.05).At the same time,the incidence of air leakage in the observation group(3.03%)was significantly lower than that in the control group(15.15%),and the difference was statistically significant(P < 0.05).Conclusion : Low pressure laryngeal mask inflation not only ensures the safety and effectiveness of anesthesia ventilation,but also reduces the incidence of postoperative complications such as throat discomfort,sore throat and stomach distension,which is conducive to promote the rapid recovery of patients after operation. |
