Development And Application Of Supportive Care Need Scale For Patients With Maintenance Hemodialysis

Objective1 To prepare a supportive care needs scale for maintenance hemodialysis patients,test the scale reliability and validity,and provide a more targeted assessment tool for studying the supportive care needs of maintenance hemodialysis patients.2 To apply a compiled scale to investigate the status of the demand and analyze the impact of general data and disease-related data on maintenance hemodialysis patients on their level of supportive care needs.Methods1 Scale preparation Guided by the theory of supportive care needs,combined with China’s cultural background,through the literature review,semi-structured interviews,two rounds of Delphi expert consultation to form a pretest scale for supportive care needs of maintenance hemodialysis patients.320 maintenance hemodialysis patients from three top three hospitals in Tianjin were selected for scale test to form a formal scale2 Scale apply A total of 241 maintenance hemodialysis patients from three top three hospitals in Tianjin were selected for investigation.The t-test,analysis of variance,and linear regression analysis were used to investigate the differences in demographic data and disease-related data of maintenance hemodialysis patients in their supportive care needs.Results1 Scale preparation(1)The Supportive Care Needs Scale for Maintenance Hemodialysis Patients ultimately contains 36 items in 7 dimensions of physical,psychological,social,emotional,spiritual,information,and practical needs.(2)The I-CVI is between 0.80 and 1,and the S-CVI is 0.95.The total score of the pre-test scale and the scores of each dimension are positively correlated(r=0.449～0.804),The total score of the clinical test scale was positively correlated with the total score of the HADS scale(r=0.407);The pre-test scale can better distinguish the needs of patients with different economic burden and different dialysis time;the exploratory factor analysis extracts 7 common factors,the load factor of each item is greater than 0.4,and the cumulative contribution rate of variance is 75.53%.(3)The Cronbach’s α coefficient for this study is 0.945,the Split-half reliability for this study is 0.857;the Cronbach’s α coefficient of 7 dimensions is 0.891～0.937,and the half-fold reliability is 0.813～0.941.2 Scale apply(1)The total supportive care needs of maintenance hemodialysis patients was(108.22±15.10),the score ranged from 36 to 180,and the entries were divided into(3.01±0.42).Each dimension scores moderately and needs help.The highest score is the social demand dimension,followed by the psychological demand dimension.(2)Age,payment of medical expenses,financial burden,dialysis time,and type of comorbidity can affect the level of supportive care needs of maintenance hemodialysis patients.Conclusion1 The supportive care needs scale for maintenance hemodialysis patients prepared in this study is reliable and can assess the supportive care needs of maintenance hemodialysis patients.2 In this study,the supportive care needs of maintenance hemodialysis patients were at a medium level,influenced by many factors,such as age,payment method of medical expenses,economic burden,dialysis time,and types of comorbidities.
