OBJECTIVE: The purpose of this retrospective study was to evaluate the recent results and prognostic factors affecting the outcome of arthroscopic saucerization for discoid lateral meniscus(DLM).METHODS: Fifty-four patients who arthroscopic diagnosed as a lateral discoid meniscus and underwent arthroscopic saucerization for lateral discoid meniscus(DLM)between January 2016 and August 2017 in Chengdu Sport Institute Affiliated Sports Hospital.There were 15 male and 39 female patients with a mean age of 38.4(range10-71)years at the time of operation.Of the 54 patients,22 had traumatic history,17 had non-invasive history.The symptoms were present for a median of 2 months(range 1day-10years)before the operation.According to watanabe classification,37 patients had type I(complete)discoid meniscus and 17 patients had type II(incomplete)discoid meniscus.All patients were followed up with a mean of 13.5(range6-25)months using Lysholm knee score、JOA meniscal score,VAS pain score,and Ikeuchi knee score.We analyzed the preoperative and postoperative scores to evaluate the clinical outcome of the arthroscopic treatment of discoid lateral meniscus.Analyzed the relationship between the outcome variable(Ikeuchi knee score at the final follow-up)and the predictor variables(age,gender,BMI,duration of symptoms,history of trauma,type of DLM,type of DLM tear,site of DLM tear).Results: After arthroscopic saucerization for lateral discoid meniscus,thirty-four excellent,fifteen good,five fair and zero poor results have obtained,the excellent and good rate was 90.7%.The mean preoperative Lysholm score and JOA score was(36.76±15.314)and(35.37±10.226)points and increased to(88.69±8.75)and(85.28±10.298)points at the final follow-up(P=0.0001).The VAS pain score was statistically significant from preoperative(4.94±1.338)points to(0.96±0.99)points at the final follow-up(P=0.0001).Regression analysis showed that age and duration of symptoms were predictors of the Ikeuchi score at the final follow-up(p<0.05).Conclusion: This study demonstrated that arthroscopic saucerization is an effective and safe treatment for DLM.Furthermore,age of the patient and duration of symptoms were shown to be important prognostic factors over the final clinical outcome.An excellent or a good result can be expected when the patient is young at the time of operation,or as soon as possible after the discoid meniscus injury.
