Analysis Of Related Factors For The Risk Of Lymph Node Metastasis In Clinical Stage I Non-small Cell Lung Cancer

Objective: To investigate the risk factors of lymph node metastasis in clinical stage ? non-small-cell lung cancer.Methods: We retrospective reviewed 305 patients with clinical stage ? non-small-cell lung cancer who accepted lobectomy with systematic lymph node dissection.We analyzed the risk factors of lymph node metastasis with clinical characteristics and pathological factors,including age,gender,size of tumor,histological subtype,differentiation,angiolymphatic invasion and neural invasion.Among the cases,207 had no lymph node metastasis(control group),and the other 98 had lymph node metastasis(observation group).The relationships between these clinicopathological factors and lymph node metastasis were analyzed by logistic regression analysis.Result: Univariate logistic regression analysis revealed that age(P=0.017),gender(P=0.041),tumor size(P<0.001),histological subtype(P=0.006),differentiation(P=0.002),bronchus invasive(P=0.001),angiolymphatic invasion(P<0.001)and neural invasion(P<0.001)were risk factors for lymph node metastasis,and multivariate logistic regression analysis indicated that age(P=0.002),tumor size(P=0.004),differentiation(P=0.007)and angiolymphatic invasion(P=0.006)and neural invasion(P=0.04)were independent risk factors for lymph node metastasis.Conclusion: There are various factors influencing lymph node metastasis in early stage lung cancer,and patients with younger age,bigger tumor sizer,poorer differentiation,angiolymphatic invasion and neural invasion are with higher probability of lymph node metastasis,among whom lobectomy with complete lymph node dissection should sequentially be the standard treatment.
