ObjectiveThrough investigation,the correlation between the degree of TCM syndrome differentiation,staging,esophageal and gastric varices is discussed.To clarify the internal relationship between the hypoglossal complex and cirrhosis of liver cirrhosis and provide an objective basis for the diagnosis,treatment and prognosis of cirrhosis.MethodsTo collect 100 patients who met the case inclusion criteria in the department of our hospital,and fill out the questionnaire on the premise of the patient to sign the informed consent form.For each patient,the hypopharynx appearance was recorded from the following seven aspects:dark purple color,trunk branch,trunk filling degree,varicose degree,sublingual contact,sublingual spot,length and width,and sublingual collaterals Comprehensive quantitative points;There are 6 TCM syndrome types.According to the clinical observation table,two experts from the vice high above can determine the syndrome differentiation criteria;A special person was responsible for the gastroscopy of each patient,and graded the performance of esophageal varices from four levels:none,mild,moderate,and severe.SPSS 22.0 statistical software was used for data analysis.Count data were analyzed using x2 test.Rank data was compared with rank sum test.Measured data are expressed as mean ± standard deviation,using variance analysis or t-test,variance is not used,rank sum test The degree of association and direction of the two variables were analyzed by rank correlation(rank correlation)using non-parametric statistical methods.Test level ? ?0.05,P ?0.05 was statistically significant.Result1.Among the 100 patients collected,the average score of sublingual collaterals in patients with liver qi stagnation syndrome was 16.2212.65 points,the water dampness internal resistance syndrome was 22.46±3.18 points,the damp-heat accumulation syndrome was 18.50±3.13 points,and the liver-kidney yin deficiency syndrome 20.36±3.20,spleen and kidney yang deficiency 23.43±4.12,phlegm and blood stasis syndrome 25.58±2.67 points.The mean value of the sublingual collaterals of each syndrome was as follows:liver-qi stagnation syndrome,damp-heat accumulation syndrome,liver-kidney yin deficiency syndrome,dampness internal resistance syndrome,spleen-kidney yang deficiency syndrome,and blood stasis syndrome.Among 100 patients.18 cases of liver qi stagnation syndrome,accounting for 18%of the total number,the mean value of the sublingual collaterals was the smallest;19 cases of stasis and blood stasis syndrome,19%of the total number,the mean of the sublingual collaterals maximum.After analysis of variance,the difference in sublingual collateral scores for each syndrome was statistically significant(F=23.639,P=0.000?0.05),the sublingual collateral scores of the six groups were different or not identical.After further comparing the differences in sublingual collateral scores between the two syndrome groups,the results suggested that there was no significant difference between the water dampness internal resistance syndrome and the syndromes of liver-kidney yin deficiency and spleen-kidney yang deficiency(P values were 0.090,0.492,all greater than 0.05),There were no significant differences between the damp-heat accumulation syndrome and the syndromes of liver-kidney yin deficiency,spleen-kidney yang deficiency and blood stasis syndromes(P values were 0.078,0.107,all greater than 0.05).Differences by variance analysis were statistically significant(P values were less than 0.05).2.The average of sublingual collaterals in patients with compensated cirrhosis was 17.24±3.18,and the average of sublingual collaterals in decompensated patients was 22.41± 3.84.The average of sublingual collaterals in decompensated patients was 2.The score is significantly higher than the compensatory period group.After the rank sum test,the difference in sublingual collateral scores between the two groups was statistically significant(z=-5.669,P=0.000<0.05).Using rank correlation analysis,the correlation between liver cirrhosis staging and sublingual collateral scores was analyzed,and the correlation coefficients ?k and ?swere 0.493 and 0.570,respectively,and both P values were 0.000<0.05,suggesting that both There is a linear correlation between the two,and the degree of the relationship is moderate.It shows that with the progress of liver cirrhosis,the period from the compensatory period to the decompensation period,the sublingual collateral points will also increase.3.The average scores of sublingual collaterals in the esophagogastric no-varicose vein group,mild varicose veins,moderate varicose veins,and severe varicose veins were 15.56±2.03,18.21±1.48,22.64±2.57,and 24.56±2.69.As the extent of esophageal varices increases,so does the number of sublingual collaterals.According to the rank sum test,the mean values of the esophagogastric varices indexing group and the sublingual collaterals scores of the four groups were 17.02,34.45,66.16,and 77.52,respectively.The difference was statistically significant(x2=77.297,P=0.000<0.05),indicating that the sublingual collateral scores of the 4 groups are different or not identical;After further analysis and comparison of sublingual collateral scores between any two groups,the sum of scores of sublingual collaterals between any two groups in the non-varicose,mild varicose,moderately varicose,and severe varicose groups was examined by the rank sum test.Both were statistically significant(P values were<0.05),indicating that the scores of sublingual collaterals in any two or two esophageal varices scoring groups were not the same.The correlation between esophagogastric varices and sublingual collateral scores was analyzed by rank correlation analysis.The correlation coefficients ?K and ys were 0.759 and 0.865,respectively.The bilateral P values were both 0.000<0.05,suggesting a linear correlation between the two.Relationships are highly positively correlated.When the degree of esophageal varices is heavier,the sublingual collaterals will increase.conclusionThe hypoglossal complex can be regarded as the objective index of TCM syndrome differentiation of cirrhosis.There was a significant correlation between the integration of sublingual collaterals and the stage of liver cirrhosis and the degree of esophageal and gastric varices.The higher the integral quantitative score of sublingual collaterals,the heavier the extent of esophageal and gastric varices,and the greater the severity of cirrhosis.weight.The sublingual colposcopy diagnosis method is non-invasive,low-cost,simple and easy to perform,can be repeated,and has high clinical applicability.It is a reliable method for monitoring changes in cirrhosis conditions,predicting disease progression,and predicting prognosis,and can be further promoted clinically in the future.
