ObjectiveTo investigate the clinical significance of Revolution CT low-tube voltage,low-contrast-dosage combined with ASiR(Adaptive Statistical Iterative Reconstruction)in the diagnosis of acute chest pain by comparing the effects of two contrast doses on the image quality in the triple-rule-out scan.Materials and Methods30 patients with acute chest pain underwent Revolution CT.All the patients were randomly divided into two groups according to different contrast dose.Each group contend 15 cases.Group A:Contrast agent dose 1.Oml/kg,and Group B:Contrast agent dose 0.8ml/kg.The scanning conditions are all 100Kv.The tube current adopts Smart-mA smart milliamp technology,ASiR 50%,the detection widt.h is 120-160mm,and the double-barrel high-pressure injector is used for scanning.It passes the right side at a rate of 5.Oml/s.Contrast medium was injected intravenously in the middle of the elbow,and normal saline was injected at the same rate after injection.The ROI is located at the bifurcation of the main pulmonary artery.The original image was transmitted into the ADW 4.6 workstation and performed with post-processing of volume rendering(VR),curved planar reformation(CPR),multi-planar reconstruction(MPR),and maximum intensity projection(MIP),and then the qualities of pictures were subjective and objective given marks and radiation does were statistically analyzed.Result11 patients with the stenosis of coronary artery more than>50%were found among 30 patients;pulmonary embolism 4 cases;2 cases with aortic dissection;1 case with abnormal coronary artery;1 case with ascending aortic aneurysm.Thirteen patients with chest pain had no obvious angiogenic lesions.There was no significant difference in subjective scores between A and B groups(P>0.05).There was no significant difference in enhanced CT values between ascending aorta,pulmonary artery trunk,and three branches of coronary arteries(RCA,LAD,and LCX)(P>0.05).Image noise and radiation dose had no statistically significant(P>0.05).Conclusion1.The one-time examination of triple-rule-out that can complete the simultaneous imaging of pulmonary artery,coronary artery,and aorta with one contrast injection and one scan.It can detect the major vascular lesions of the heart and can also treat the lungs and bones.The preliminary screening of the chest and surrounding soft tissue lesions has important clinical significance for the etiological screening of patients with vascular and non-vascular acute chest pain.2.The triple-rule-out with 1.Oml/kg and 0.8ml/kg of contrast agent can clearly show the pulmonary artery,coronary artery and aortic vascularity.The image quality can meet the clinical diagnosis requirements,but 0.8ml/kg contrast agent dosage lower,reduce the contrast agent’s toxic effects on the kidneys,and provide safer examination methods for children and patients with renal insufficiency.3.This studies showed that the Revolution CT 100KV low-tube voltage technology combined with ASiR 50%reconstruction algorithm,using a lower contrast injection protocol(contrast agent dosage 0.8ml/kg,injection speed 5ml/s),can not only Reducing the contrast agent dosage of the subject,but effectively reduce the radiation dose,and the image quality can meet the needs of clinical diagnosis,and provide an efficient inspection method for guiding clinical early intervention treatment,with high clinical value. |
