BackgroundWith the progress of science and technology and radiation medicine,people have more and more exposure to ionizing radiation.While bringing convenience to people's lives,ionizing radiation also has a negative impact on the human body.Reproductive system is very sensitive to ionizing radiation,especially testis.Very low radiation dose can destroy the function and structure of testis,resulting in the reduction of sperm number and androgen synthesis.Traditional Chinese medicine has a certain protective effect on testicular radiation injury,but its research is still less,mostly focused on monomer of traditional Chinese medicine and a few compound prescriptions,which can not meet the clinical needs.ObjectiveAccording to the etiological characteristics and main clinical manifestations of ionizing radiation and the basic theory of traditional Chinese medicine,this paper summarizes the pathogenesis of male reproductive injury caused by ionizing radiation,and provides a theoretical reference for the syndrome differentiation and treatment of testicular radiation injury.At the same time,the clinical manifestations of male reproductive system injured by ionizing radiation,combined with its etiological and pathogenic characteristics,summarizes its medication rules for clinical and clinical use.Provide ideas for scientific research and drug use.The effects of ionizing radiation on acute radiation damage of testis and the protective effect of Chinese herbal medicine compound were observed in the experimental study,and whether it played a role by affecting TLR5/NF?B signal pathway was discussed.MethodTheoretical research:Combining the characteristics of etiology and treatment of ionizing radiation in traditional Chinese medicine,the pathogenesis of reproductive system injury caused by ionizing radiation is discussed;combing the law of medication for oligoasthenospermia,combining the etiology and pathogenesis of male reproductive injury caused by ionizing radiation,providing a reference for the medication of testicular radiation injury caused by ionizing radiation.experimental studyMale Balb/c mice were randomly divided into normal group,model group,positive drug group,high-dose group and low-dose group according to their body weight.Normal mice were given blank control group.The other four groups were given one-time 2.0 Gy 60Co gamma-ray whole body irradiation.Before and after irradiation,the overall condition of mice,such as fur,mental state,fecal traits,diet and activity,were observed at any time.The body weight and testicular index of mice were recorded,and the weight change curve of mice was drawn.The following experiments were carried out 10 days after administration and 1,3 and 7 days after irradiation.Experiment 1:The changes of body weight and testicular index of mice in different time groups were detected,and the pathological changes of testicular structure of mice in different time points were observed and semi-quantitatively analyzed.Experiment 2:To detect the apoptotic rate of spermatogenic cells and the expression of mitochondrial apoptotic genes Bcl-2,Bax,Cytochromec and Caspases-3 at different time.Experiment 3:The expression of TLR5/NF? B signaling pathway-related factors and the expression of TLR5 and NF-kappa B in testis of mice at different time were detected.ResultTheoretical part:When male reproductive injury is caused by ionizing radiation,kidney essence is depleted and spermatogenesis is lack of source;spleen is out of health,Qi and blood are lack of source;at the same time,ionizing radiation poisonous pathogen is fierce,Qi is depleted and Yin is injured,which not only depletes the essence of reproduction,but also makes the kidney lack of nourishment and refinement and infertility,but also makes the kidney lose its "working strength",tendons lose in moistening,while the penis wiltered and not used for a long time,resulting in sexual dysfunction.In the treatment of oligospermia and asthenospermia,the main medicines are tonics,firming the essence and shrinking the urine and stopping the bandage,diuresis and dampness-promoting drugs,heat-clearing drugs and Blood-activating and stasis-removing drugs;from the point of view of frequency of medication,high-frequency Chinese medicines are mainly tonics,including tonifying yang,yin,Qi and blood,mostly into the kidney meridian,with Gan and Wenping as the main sexual flavor.In drug compatibility,attention should be paid to Yin-Yang compatibility,Qi-blood coherence,reinforcing and reducing simultaneously,which corresponds to its complex and changeable pathogenesis.Experiment 1:The body weight of mice decreased slowly on the 2nd day after irradiation and began to rise slowly.The morphological structure of testis was damaged and the Johnsen score of testis in the model group decreased gradually with the change of irradiation days.Compared with the model group,the traditional Chinese medicine compound can effectively promote the weight recovery of mice after irradiation and protect testis from pathological changes.Experiment 2:The apoptotic rate of spermatogenic cells increased significantly after irradiation.The expression of apoptotic related genes:Cytochrome C,Caspases 3 and Bax increased,while the expression of Bcl-2 decreased.Compared with model group,traditional Chinese medicine compound could effectively reduce the apoptotic rate of spermatogenic cells after irradiation,reduce the expression of Cytochrome C,Caspases 3 and Bax,and increase the expression of Bcl-2.Experiment 3:According to the results of immunohistochemistry,there was no significant difference in the expression of TLR5 and NF ? B between the groups before irradiation and the normal group;one day after irradiation,the expression of TLR5 in each group was higher than that in the normal group and the model group;and the expression of NF?B in the high dose group was higher.Three days after irradiation,compared with the normal group and model group,the expression of TLR5 increased in the positive group and the low dose group,and the expression of NF?B increased in the low dose group and the high dose group.Seven days after irradiation,the expression of TLR5 and NF ? B in each irradiation group was lower than that in the normal group,while the expression of TLR5 and NF ? B in the high-dose group and the low-dose group was higher than that in the model group.Conclusion:Theoretical research:The pathogenesis of male reproductive injury caused by ionizing radiation may be in three aspects:1.Toxic pathogen is very strong,kidney essence is deficient;2.Spleen is out of health,Qi and blood are lack of source;3.Toxic pathogen is very strong,consuming Qi and damaging Yin.In terms of specific drug use,we can draw lessons from the law of oligoasthenospermia,combined with the etiology and pathogenesis of reproductive system caused by ionizing radiation,which is conducive to better treatment of reproductive function damage caused by ionizing radiation in clinic.Experiments:The acute testicular injury model of mice was successfully reproduced in this experiment.Chinese herbal medicine compound,especially low dose,played a good protective role,which may be achieved by activating TLR5/NFkappa B pathway to reduce spermatogenic cell apoptosis.
