| Objective:In this study,Ba Duan Jin combined with Cognitive Behavioral Therapy was used to intervene in elderly insomnia to study the effect of this comprehensive therapy on sleep quality,quality of life,anxiety and depression in elderly patients with insomnia.Through this study,we assessed the compliance and safety of the therapy,explored the chronic disease management program for elderly insomnia.Methods:In this study,prospective randomized controlled clinical trials were used to recruit 74 elderly patients with insomnia who met the inclusion criteria.The 74 research objects were randomly divided into control group and test group,37 cases in each group.The test group was treated with Ba Duan Jin combined with Cognitive Behavioral Therapy and the intervention method of control group was walking combined with general health education.The Pittsburgh Sleep Quality Index(PSQI),Self-rating Anxiety Scale(SAS),Self-rating Depression Scale(SDS),and the Short-Form 36 Health Survey Scale(SF-36)were used at 0,2,4,6,and 8 weekends during the intervention to evaluate the effect of this comprehensive therapy on sleep quality,quality of life and psychological intervention in elderly patients with insomnia,and according to diagnostic criteria for insomnia in the Guiding Principles for Clinical Research of New Drugs in Traditional Chinese Medicine issued by the Ministry of Health to evaluate its overall efficacy.Results:1.The results of baseline assessment:After 8 weeks of comprehensive intervention,the control group shed 8 people and the remaining 29 patients.The text group shed 5 people and the remaining 32 patients.There was no statistical difference between the two groups in the number of the shedding and completion(P>0.05),and they were comparable.There were no significant differences in gender,age,education,and duration of disease between the two groups(P>0.05).And there were no significant differences in the scores of PSQI,SAS,SDS and SF-36 between the two groups before the rtreatment(P>0.05),which was comparable.2.The results of efficacy assessment:After the intervention,the control group recovered 1 case,markedly effective in 3 cases,effective in 11 cases,14 cases were ineffective,the total effective rate was 51.72%;the text group recovered in 6 cases,markedly effective in 10 cases,effective in 11 cases,5 cases were ineffective,total effective rate was 84.38%;the difference in the overall efficacy evaluation between the two groups was statistically significant(P<0.05).3.The results of sleep quality assessment:(1)The score situation of PSQI:At the end of the second week,the difference in daytime dysfunction between the control group and the test group was statistically significant(P<0.05).At the end of the fourth week,compared with week 0,the test group had statistically significant differences in sleep quality and total score of PSQI(P<0.05).At this time,there was also a statistically significant difference between the control group and the test group in terms of sleep disturbance and daytime dysfunction(P<0.05).At the end of sixth week,compared with week 0,the test group had statistically significant differences in the sleep quality,sleep latency,sleep time,sleep efficiency,sleep disturbance,daytime dysfunction and total score of PSQI(P<0.05).And meanwhile,there were significant differences in sleep disorders,daytime dysfunction and total score of PSQI between the control group and the test group(P<0.05).At the end of eighth week,compared with week 0,the control group had statistically significant differences in daytime dysfunction(P<0.05)while the test group had statistically significant differences in all factors of PSQI as well as total score(P<0.05).And at the same time,there were significant differences in sleep quality,sleep time,sleep efficiency,sleep disturbance and total score of PSQI between the control group and the test group(P<0.05).(2)Comparison of hypnotic drug reduction rate:The reduction rate of hypnotic drugs in the control group was 27.78%while the test group was 68.00%.There was a statistically significant difference between the two groups after treatment(P<0.05).4.The assessment results of quality of life:At the end of the second week,there was a statistically significant difference in role limitations due to emotional problems(RE)between the control group and the test group(P<0.05).At the end of the fourth week,compared with week 0,the test group had statistically significant differences in reported health transition(HT)(P<0.05).And meanwhile,there were significant differences in RE and HT of SF-36 between the control group and the test group(P<0.05).At the end of sixth week,compared with week 0,the test group had statistically significant differences in general health perceptions(GH),vitality(VT)and HT(P<0.05).At the same time,there were also statistically significant differences in role limitations due to physical health(RP),GH,VT,RE and mental health(MH)between the control group and the test group(P<0.05).At the end of eighth week,compared with week O,the test group had statistically significant differences in RP,bodily pain(BP),GH,VT,social functioning(SF),RE,MH and HT(P<0.05).And meanwhile,there were significant differences in RP,BP,GH,VT,SF,RE,MH and HT between the control group and the test group(P<0.05).5.The assessment results of psychological intervention:(1)The score situation of Self-rating Anxiety Scale:There were significant differences in SAS scores between the control group and the test group at the end of sixth and eighth week(P<0.05).The SAS scores of the test group at the end of fourth,sixth,and eighth weekends were significantly different from those of week 0(P<0.05).(2)The score situation of Self-rating Depression Scale:The SDS scores of the test group at the end of sixth and eighth weekends were significantly different from those of week 0(P<0.05).Conclusion:1.Ba Duan Jin combined with Cognitive Behavioral Therapy in the treatment of elderly patients with insomnia was better than walking combined with general health education.2.Ba Duan Jin combined with Cognitive Behavioral Therapy could improve sleep quality,sleep efficiency,reduce the time taking to fall asleep,prolong total sleep time,alleviate sleep disorders,daytime dysfunction,reduce the use of hypnotic drugs in elderly patients with insomnia.This therapy was effective in improving the physiological,social,emotional,and general health of elderly patients with insomnia,alleviating somatic pain and improving energy.The therapy could also improve anxiety and depression in elderly patients with insomnia,help maintain mental health,and promote overall health in a good direction. |
