| Objective:The annual incidence of idiopathic myopathy is about 1/1000000.,in which the incidence of interstitial lung disease is 23.1-78%and 5 years’ mortality rate 50%.The treatment of this disease is mainly based on hormone combined with immunosuppressive agents.The short-term effective rate is only about 50%and is easy to relapse in the course of treatment.what’s more,the long-term effect is not accurate and the clinical data of this disease are only reported in the case.The purpose of this study is to explore the characteristics of traditional Chinese medicine about the disease,summarizing the scheme of TCM syndrome treatment.Methods:The cross-sectional study combined with case follow up were used to collect the patients diagnosed as IIM-ILD in the Department of.Beijing TCM hospital from January 2016 to march 2018.The basic clinical information,image data,lung function,and information of Chinese medicine were collected by the main doctor.The main syndrome types were identified and their prescriptions were analyzed by chief physician.Finally,the results were recorded in the database and analyzed statistically by the researcher.Results:1,Among 65 cases of IIM-ILD patients,there were 17 males and 48 females,including 12 cases of rapid progression and 53 cases of chronic stage.There were 5 case of PM-ILD,17 case of DM-ILD,40 case of ASS and 3case of CADM-ILD.2,12 cases of rapid progress period all showed dyspnea,5 cases of muscle weakness,4 cases of rash,6 cases of respiratory failure about the clinical manifestation.In term of HRCT,OP+NSIP was mainly type.The positive rate of anti ARS antibody in serology is highest in this disease.Among 53 cases of IIM-ILD patients,the main manifestations were artisan hand Gottron sign and muscle weakness in different degrees.On the other hand,NSIP.was main type in HRCT.3,In the term of statistics about traditional Chinese medicine information,the rate of rapid progress sthma is 100%,the proportion of muscle flaccid weakness is 41%,tongue red,moss yellow,and greasy are all 50%.The proportion of the pulse skater was 41%.In term of the chronic period,the suffocate is 49.06%,the cough is 43.40%,the phlegm and the sticky is 43.40%,the phlegm is 24.53%,the mouth greasy is 52.83%,the head sweat is 37.74%,the symptom of dry mouth as well as drinking little is 20.75%,the limbs weak and heavy is 18.87%,the joint swelling is 15.09%,the loose stool is 13.21%,the stool is 50.94%,the tongue red is 32.08%,the yhuang is 56.60%and the veins is 8%,the pulse number is 52.83%.Conclusions:1,The main syndromes are damp heat leepy lung spleen-disturbancing blood stasis,damp heat stagnation of lung and spleen in rapid progress.The main syndrome types contain damp retention,lung and spleen deficiency,dampness heat blockage,channels and collaterals failure,lung heat and spleen cold,lung and spleen qi deficiency in the chronic phase.2,The etiology and pathogenesis analysis:Damp heat evil is the main in this disease.The rapid progress period is mainly caused by the invasion of damp heat,involving the skin,muscles and lungs,disturbing the blood of the camp.In terms of chronic phase,stubborn wet resistance,battalion blood stagnation were easy to been seen.3.The records of treatment:In terms of rapid progress period,clearing and damping heat are main in the rapid progress period,"Xuan Chang Qi Ji and Cool blood and Detoxification,and then Damp heat and Yin,"are the main treatment.In terms of chronic phase"Supplementing qi and fixation,Supplementing lung and spleen,and Clearing heat and dampness" are main treatment.4.In term of the effect of treating,the only Chinese medicine scheme or the combination of traditional Chinese with western medicine can improve the patients’ asthma,cough,rash,mechanic hand and muscle weakness through the follow-up observation of the patients.What’s more,It is possible to reduce the recurrence of the disease and help to reduce the hormone. |
