1 Objective(1)To study the expression of serum Klotho protein and FGF23 in patients with MHD and CAPD,and to explore their correlation with calcium and phosphorus metabolism in dialysis patients. It is expected that Klotho protein and FGF23 should be used as slow renal failure minerals and bone metabolism(CKD-MBD).The experimental diagnostic markers and therapeutic targets provide experimental basis and theoretical basis;(2)To study the distribution of TCM syndromes in patients with MHD and CAPD,and explore the correlation between serum klotho protein,FGF23 and TCM syndromes, and expect to be soluble in serum. The content of klotho protein and FGF23 is one of the basis for the syndrome differentiation and pathogenesis of chronic renal failure.2 MethodThis study adopts a cross-sectional survey method to sign informed consent for patients who meet the inclusion criteria and agree to participate in the study, fill in TCM syndrome data, and judge TCM syndromes. The levels of serum klotho protein and FGF23 in dialysis patients were determined by enzyme-linked immunosorbent assay(ELISA),and the basic information of patients was collected. The medical history was collected, and the blood routine, liver function, renal function, ions and serum albumin were collected.Statistical methods were used to analyze the correlation between serum klotho protein,FGF23 and calcium and phosphorus metabolism, and TCM syndromes in dialysis patients.3 Result(1)Correlation between serum klotho protein,FGF23 and calcium and phosphorus metabolism:serum FGF23 is positively correlated with calcium, phosphorus, calcium and phosphorus, and parathyroid hormone in patients with MHD;serum FGF23 and phosphorus, calcium and phosphorus products, parathyroid hormone in patients with CAPD There was a positive correlation between serum klotho protein and calcium and phosphorus metabolism in two dialysis modes;(2)Comparison of serum klotho protein and FGF23 in two dialysis methods:serum klotho protein in MHD group was lower than CAPD group,FGF23 was higher than CAPD group;(3)Distribution pattern of TCM syndromes under two dialysis methods:MHD group deficiency syndrome is mainly caused by liver and kidney yin deficiency, and standard evidence is mainly blood stasis syndrome;CAPD group deficiency syndrome is mainly spleen and kidney qi deficiency. The standard evidence is mainly wet and turbid;(4)Correlation between serum klotho protein,FGF23 and TCM syndromes:In this deficiency syndrome, there were significant differences in serum klotho protein levels between the spleen and kidney qi deficiency group, the liver and kidney yin deficiency group, the spleen and kidney yang deficiency group, and the qi and yin deficiency group. The spleen-kidney qi deficiency group had the highest klotho protein content;in the standard evidence, the serum klotho protein content of the Shuiqi syndrome group, the damp-heat syndrome group and the blood stasis syndrome group were significantly different, and the water-gas syndrome group had the highest klotho protein content; There was no significant difference in the content of FGF23 between the groups and the evidence.4 Conclusions(1)Serum FGF23 in dialysis patients is closely related to calcium and phosphorus metabolism, and positively correlated with calcium, phosphorus, calcium and phosphorus, and parathyroid hormone;(2)there are differences in serum klotho protein and FGF23 between the two dialysis methods;(3)Under the two dialysis methods, the distribution patterns of TCM syndromes are different;(4)There is a certain correlation between serum klotho protein and FGF23 in dialysis patients and TCM syndromes, which has reference significance in guiding syndrome differentiation.
