Clinical Observation Of Taohong Siwu Decoction And Fangji Huangqi Decoction In Preventing Deep Vein Thrombosis After Intertrochanteric Fracture

Objective:Using TaoHongSiWu Decoction and FangJiHuangQi Decoction to intervention the elderly intertrochanteric fracture of the femur after the operation,,exploring the results of using this decoction to prevent the elderly intertrochanteric fracture of the femur patients' deep venous thrombosis.Methods:From March 2018 to March 2019,80 patients with intertrochanteric fracture of femur,aged 60 to 95 years,from Department of Trauma,Changde Hospital affiliated to Hunan University of Traditional Chinese Medicine,were randomly divided into observation group and control group.All the patients were operated within 3 days after injury.The control group received hypodermic injection of low molecular weight heparin calcium 4000 U Qd to 12 hours before operation and continued routine anticoagulation 12 hours after operation.In the observation group,the dose of low molecular weight heparin calcium was changed to 2000 U after operation,also TaoHongSiWu Decoction and FangJiHuangQi Decoction were taken from 12 hours after operation until 14 days after operation.The incidence of DVT,the VAS score at 1st,3rd and 5th day after operation,the postoperative drainage,and the PT,APTT,PLT,D-dimer values at admission and 3rd,7th,14 th day after operation were collected and observed in the two groups.Results: There were 74 cases(37 cases in the observation group and 37 cases in the control group)that could be carried out according to the test scheme.There was No deep venous thrombosis happened in the two groups,There was no significant difference in age,sex and fracture type between the two groups.1.VAS score:both groups could improve the postoperative pain(P < 0 05),and the VAS score of the observation group on the 3rd and 5th day after operation was significantly lower than that of the control group(P < 0 05).2.Postoperative drainage: The postoperative drainage volume in the observation group was less than that in the control group(P < 0.05).3.PT APTT: PT: both groups could improve the postoperative PT value(P < 0.05),and the curative effect of the two groups was the same(P > 0.05).4.D-dimer: the value of D-dimer in the two groups decreased from 3 to 7 days after operation(P < 0.05),and decreased to the same level as that before operation on the 14 th day after operation,and the curative effect of the two groups was the same(P > 0.05).5.PLT value:the treatment regimen of both groups resulted in platelet decrease(P < 0.05),but the extent of platelet decrease was smaller in the tube scrub group on the 7th day and the 14 th day after operation(P < 0.05).Conclusion:The application of TaoHongSiWu Decoction and FangJiHuangQi Decoction in patients with intertrochanteric fracture of femur after operation has a good effect on preventing deep venous thrombosis of lower extremity,and can improve the postoperative pain,reduce the blood loss after operation.
