Observation Of The Clinical Efficacy Of External Use Of Yanghe Ointment Combined With Triamcinolone Acetonide Milk Duct Irrigation On Granulomatous Mastitis In The Mass Stage

Background:The etiology of granulomatous mastitis is still unclear at home and abroad,and the pathogenesis is still unclear.There is no international standard of diagnosis and treatment.The domestic Chinese medicine mammary gland industry has not formed a mature and effective consensus on the pathogenesis and treatment principles of this disease.Therefore,it is urgent to find a treatment plan with high efficiency,small trauma,maximum retention of breast shape to alleviate the suffering of patients and low recurrence rate and small side effects.Research purposes:At present,there is no prospective research report on the topical perfusion of topical perfusion with traditional Chinese medicine.Based on previous studies,this study designed a cohort study to compare the clinical efficacy of external application of Yanghe cream with triamcinolone acetonide and simple triamcinolone acetonide for local clinical treatment.Provide evidence-based medical evidence.Research methods:According to the relevant principles of evidence-based medicine,the study was conducted by collecting the breast department of Dongzhimen Hospital of Beijing University of Chinese Medicine from September 2016 to November 2018,and diagnosed as the initial stage of granulomatous lobular mastitis(the mass stage).Patients were randomized into groups of 30 each.Experimental group:external application of Yanghe cream combined with triamcinolone acetonide.Control group:simple triamcinolone acetonide rinse.The course of treatment was 1 month.By comparing the improvement of symptoms such as breast mass size,breast pain score,breast skin temperature and breast mass hardness before and after treatment in the experimental group and the control group,Yanghe cream combined with triamcinolone acetonide were evaluated.The clinical efficacy and effectiveness of breast tube irrigation in the treatment of mass granulomatous mastitis.Results:1.General data analysis:A total of 60 patients with granulomatous mastitis were enrolled in this study.The largest age was 41 years and the youngest was 25 years old.The average age of onset was(32.9±3.56)years old.Of the 60 cases,only 2 were unmarried women,only 1 of 60 ccases were unproduced,and the remaining 59 cases were maternal.The shortest postpartum period is 0 months,the longest is 12 months,and the incidence is higher after 4-5 months postpartum.The minimum time for breastfeeding is 0 months,the longest is 36 months,with an average of(15.9±8.3)months.The course of disease was as short as 7 days and the longest was 18 days.The average duration of disease was(12.0±7.0)days.Among them,9(15%)had a history of breast trauma,and 9(15%)had a history of taking contraceptives.After statistical analysis,there were no significant differences in the age,maternal,breastfeeding time,postpartum time,duration of disease,history of breast trauma,and whether or not to take birth control pills(P>0.05).2.Baseline analysis of the efficacy index of the two groups before treatmentThere was no significant difference between the control group and the experimental group in the maximum diameter of the mass before the drug intervention,the skin temperature of the affected area,the pain score,and the breast mass hardness(P>0.05).3.Analysis of results of efficacy indicators(1)Analysis of differences before and after treatment in the groupAfter the drug intervention,the maximum diameter of the tumor in the control group and the experimental group,the skin temperature of the affected area,the pain score level,and the mass of the mass were significantly lower than those before the treatment(all P<0.001,with extremely significant statistical difference).(2)Analysis of post-treatment differences between groupsAfter drug intervention,the maximum diameter of breast mass,breast skin temperature,breast pain score,and mass of the tumor were significantly lower than the control group(P<0.05,statistically significant).(3)Total efficiencyln the control group,30 patients were cured(6%(20%),12(40%),8(27%),and 4(13%);30 patients in the experimental group,15(50%)),11 cases(37%)were markedly effective,4 cases(13%)were effective,and 0 cases(0%)were ineffective.After statistical analysis,the effective rate of the two groups was statistically different.The effective rate of the experimental group was significantly higher than that of the control group.4.Conclusion:(1)Yanghe cream topical combined with triamcinolone acetonide lavage and simple triamcinolone acetonide lavage are all granulomatous mastitis masses,breast mass,skin temperature,breast pain score,breast lumps There is a certain effect on the improvement of hardness and the like.(2)Yanghe cream topical combined with triamcinolone acetonide for the treatment of mass granulomatous mastitis is better than simple triamcinolone acetonide,which has better curative effect.(3)Yanghe cream external combined with triamcinolone acetonide for the treatment of mass granulomatous mastitis is effective,easy to operate,high in safety,and has certain clinical application and promotion value.
