| Obj ective:Through retrospective study to analyze the clinical data of patients with stage ? colon cancer,summarize the common TCM syndrome types and superior syndrome types of left and right colon cancer,and observe the influence of traditional Chinese medicine on the survival of left and right colon cancer patients.Willing to it provides a theoretical basis for TCM syndrome differentiation and treatment of patients with stage ? colon cancer.Methods:Retrospectively sorted and analyzed the medical records of 109 patients with stage ? colon cancer who were treated with integrated Chinese and Western medicine and followed by phone in the hematology-oncology department of Dongzhimen Hospital of Beijing University of Chinese Medicine from January 2007 to December 2016.Summarize the TCM syndromes of left and right colon cancer.Survival period was taken as the main observation index,compared with the survival time reported in related literatures,and the TCM syndrome types of patients with median survival time were analyzed.The product limit method(k-m)was used to analyze the relationship between the survival time of left and right colon cancer and the duration of oral Chinese medicine.Results:A total of 109 eligible cases were included in the study.Through telephone follow-up and medical records,99 patients were confirmed death,the cause of direct death was colon cancer.10 cases survived,and the percentage of censored was 9.2%.Among them,74 cases were left colon cancer.The median survival time was 22.3 months(669 days),the 1-year survival rate was 74.32%(55/74),the 2-year survival rate was 35.14%(26/74),and the 3-year survival rate was 20.27%(15/74).35 patients were right colon cancer,the median survival time was 12.4 months(372 days),the 1-year survival rate was 51.43%(18/35),the 2-year survival rate was 25.71%(9/35),and the 3-year survival rate was 8.57%(3/35).After P<0.05 by Log-Rank test,the difference was statistically significant.The median time of oral Chinese medicine decoction in the whole group was 6.00 months.The left and right colon cancer groups were divided into two subgroups of oral Chinese herbal decoction for less than 6 months and greater than or equal to 6 months according to the time of oral Chinese medicine decoction.In the left colon cancer group,there were 37 cases in both subgroups with oral Chinese medicine decoction less than 6 months and greater than or equal to 6 months,8 cases were deleted and the deletion rate was 10.8%.The median survival time of the group with Chinese medicine decoction less than 6 months was 15.8 months,the 1-year survival rate was 59.46%(22/37),the 2-year survival rate was 35.14%(13/37),and the 3-year survival rate was 21.62%(8/37).The median survival time was 22.50 months,1-year survival rate was 82.14%(33/37),2-year survival rate was 35.14%(13/37),and 3-year survival rate was 18.92%(7/37).The difference was not statistically significant by log-rank test(P>0.05).In the right colon group,16 patients and 19 patients were included in the two subgroups of oral Chinese medicine decoction less than 6 months and greater than or equal to 6 months,respectively.The number of deleted patients was 1 case,respectively,and the deletion rate was 6.3%and 5.3%respectively.The median survival period of the two subgroups was 5.1 months and 14.8 months,and the 1-year survival rate was 31.25%(5/16)and 68.42%(13/19).The difference was not statistically significant by log-rank test(P>0.05).The TCM syndrome type of stage ? colon cancer is complex,mainly due to damp-heat accumulate knot(28,25.69%),qi and blood deficiency(26,23.85%)and spleen and kidney yang deficiency(24,22.02%).? colon cancer patients with gender,age,differentiation degree,pathological type,transfer site and treatment will have no effect on colon cancer syndromes.There was a significant difference in the distribution of TCM syndrome between left colon cancer and right colon cancer(P<0.05).The TCM syndrome of left colon cancer was mainly damp and hot accumulate knot,while the right colon cancer is dominated by qi-blood deficiency syndrome.The dominant syndrome of the left colon cancer is damp and hot accumulate knot,the inferior syndrome is spleen-kidney yang deficiency.The inferior syndrome of the right colon cancer is liver and kidney Yin deficiency.Conclusion:(1)Combine traditional Chinese and western medicine treatment in improving the survival time of patients with stage ? colon cancer have certain advantages;(2)As the extension of oral Chinese medicine time,left and right half colon cancer can get longer median survival;(3)The TCM syndrome type of stage ? colon cancer is mainly caused by deficiency syndrome,mainly due to damp and hot accumulate knot,qi-blood deficiency and spleen-kidney yang deficiency;(4)There is a significant difference in the distribution of TCM syndromes between left and right colon cancer(P<0.05).The TCM syndromes of left colon cancer are mainly by damp and hot accumulate knot,while the right colon cancer is dominated by qi-blood deficiency syndrome.(5)The dominant syndrome of the left colon cancer is damp and hot accumulate knot,the inferior syndrome is spleen-kidney yang deficiency.The inferior syndrome of the right colon cancer is liver and kidney Yin deficiency. |
