AIM To investigate the characteristic phenotype of electrocardiogram(ECG)in hypertrophic cardiomyopathy(HCM)patientsMETHODS 60 inpatients with HCM from May 2015 to April 2017 in Xijing hospital were enrolled in the study.Another 60 normal people(gender and age and weight matched in Xijing hospital)were chosen as control group.The 12-leads body surface ECG were analysed,including QRS duration,R wave and S wave amplitude,abnormal q wave,duration of QTC,R/S ratio,ST segment elevation and depression,T wave low and inversion,P wave duration.RESULTS(1)In HCM group,QRS duration in V2 and V3 leads,abnormal Q wave proportion in II and V4 leads,QTC duration,P wave duration and the ECG diagnostic formula of left ventricular hypertrophy were significantly larger than those in normal control group.(2)In HCM group,QRS duration in I,a VR,a VL and a VF leads,and Q wave proportion in a VR leads,and QRS main wave direction consistent with T wave in I,II,III,a VL,a VF,V3,V4,V5,V6,and R/S ratio in V4,V5 and V6 leads were significantly smaller than those in normal control group.CONCLUSION The ECG diagnosis of HCM firstly met the standard of left ventricular hypertrophy,and then combined with the specific parameters of each of the above ECG leads.AIM Compare the differences in parameters of 12 lead routine electrocardiogram(ECG)and echocardiography between patients with hypertrophic cardiomyopathy(HCM)and patients with hypertensive left ventricular hypertrophy(H-LVH).To explore the value of ECG in detecting patients with HCM and H-LVH.METHODS This study included 3 groups HCM patients,H-LVH patients and healthy group(control group),and each group had 49 cases.ECG analyzed including P wave duration,QTC duration,QRS duration,abnormal Q wave,the amplitude of R wave and S wave.E/A,the left ventricular ejective fraction(LVEF),the maximum left ventricular wall thickness(MLVWT),left atrial diameter,Systole of left and right left ventricle?Diastolic phase of left and right left ventricle?Systolic phase of Left ventricle anterior and posterior diameter?Diastolic phase of Left ventricle anterior and posterior diameter?Systole of Long axis of left ventricle?Diastolic phase of Long axis of left ventricle were measured by conventional echocardiography.The above indexes were compared the differences between three groups.RESULTS Compared with H-LVH group,patients with HCM presented increased LVEF and MLVWT(P<0.05).However,Per stroke output?Systole of left and right left ventricle?Diastolic phase of left and right left ventricle?Systolic phase of Left ventricle anterior and posterior diameter?Diastolic phase of Left ventricle anterior and posterior diameter?Systole of Long axis of left ventricle?Diastolic phase of Long axis of left ventricle of H-LVH group were significantly higher than HCM group(P < 0.05).Compared with H-LVH,HCM patients had a higher QRS voltages in leads II,III,a VF,V1 and V2 and prolonged QTC duration(P<0.01).The abnormal Q wave proportion and QRS main wave direction consistent with T wave in leads III?a VR were higher in HCM group(P<0.01).CONCLUSION Combined with the specific parameters of QRS voltages,QTC duration,abnormal Q wave proportion in leads II,V4 and QRS main wave direction consistent with T wave in leads V2,V3,and V6 could detect H-LVH from HCM patients.
