Purpose: To investigate whether nerve conduction velocity basically returned to normal and substance P content in spinal cord and hypothalamus decreased in LDH rats by acupuncture treatment;to observe the morphological changes of L5 nerve roots before and after treatment by light microscopy,and to explore the effect of acupuncture on nerve repair;The specificity of the acupoints was further confirmed by analysis of the therapeutic effects.Material and method:40 male healthy SD rats were divided into normal group(n = 10),model group(n = 10),Shenshu group(n = 10),non-meridian and non-acupoint group(n = 10)according to random number table.In the normal group,no model was made,no acupuncture was given,and normal feeding was given.Modeling method of model group: After anesthesia,the tail was broken,the tail intervertebral disc was cut,and the nucleus pulposus was taken to make suspension.The L4-L5 spinous process space was punctured and the 9th lumbar puncture needle was used for epidural puncture.After successful puncture,the autologous nucleus pulposus suspension was injected into rats.After the model was made,the animals were fed normally.The Shenshu group was the same as the model group.Three days after the establishment of the model,the rats were acupunctured at Shenshu acupoint once a day and treated five times.The non-meridian and non-acupoint group was the same as the model group.Three days after the establishment of the model,the non-meridian and non-acupoint group was acupunctured in rats(taking the middle point of the second lumbar spinous process depression and Shenshu connection),once a day and treated five times.One day before the establishment of the model,three days after the establishment of the model and five days after the treatment,the threshold of contraction of the claw stimulated by heat and the changes of nerve conduction velocity in the model rats were measured respectively.2 MML5 nerve root tissue was taken and stained with hematoxylin-eosin(h-e)solution to observe the morphological changes of nerve root.the content of substance P(sp)was determined by enzyme-linked immunosorbent assay(elisa).Results: 1.Three days after modeling,there was a significant difference between the model group,Shenshu group and non-meridian non-acupoint group and the day before modeling(p < 0.05),suggesting that the threshold of contraction of claws was decreased after modeling,which was in line with the characteristics of lumbar disc herniation being sensitive to pain.Therefore,the model was successfully established.After five days of treatment,there was a significant difference between Shenshu group and the three days after modeling(p < 0.05),suggesting that the heat pain stimulation after treatment was significantly different(p < 0.05).The threshold of claw contraction has been raised,which indicates that acupuncture at Shenshu point can alleviate the sensitivity of rats to pain and relieve the pain of rats.2.The sensory nerve conduction velocity and motor nerve conduction velocity of rats in model group,Shenshu group and non-meridian and non-acupoint group three days after model establishment were significantly different from those of rats one day before model establishment(p < 0.05),suggesting that model establishment was successful;the sensory nerve conduction velocity and motor nerve conduction velocity of rats in Shenshu group five days after treatment were significantly different from those of rats in the same group three days after model establishment(p < 0.05).(p < 0.05),suggesting that the treatment is effective;the sensory nerve conduction velocity and motor nerve conduction velocity of rats in non-meridian and non-acupoint group after five days of treatment are significantly different from those in Shenshu group after five days of treatment(p < 0.05),suggesting that the therapeutic effect of Shenshu group is better than that of non-meridian and non-acupoint group.3.He staining of nerve root showed that regenerated myelin sheath appeared in Shenshu group compared with model group,the overall arrangement was regular,the size was the same,inflammatory cell infiltration was significantly reduced,and cell edema was not obvious;but there were still a large number of inflammatory cell infiltration in non-acupoint group compared with model group,the cell boundary was not clear,the size was different,the arrangement was irregular,and the cell edema was obvious.It is suggested that acupuncture at Shenshu acupoint can effectively alleviate the inflammatory reaction and edema of nerve root.4.The contents of substance P in the spinal cord and hypothalamus of the model group,Shenshu group and non-meridian non-acupoint group were significantly different from those of the normal group at the same location(p < 0.05),suggesting that substance P content in the spinal cord and hypothalamus of the Shenshu group were increased and the model was successfullyestablished.The contents of substance P in the spinal cord and hypothalamus of the Shenshu group were significantly different from those of the model group at the same location(p < 0.05),suggesting that substance The content of substance P in spinal cord and hypothalamus of nonmeridian and non-acupoint group was significantly lower than that of Shenshu group(p < 0.05),suggesting that the change of substance P in spinal cord and hypothalamus of Shenshu group was significantly higher than that of non-meridian and non-acupoint group,proving that the effect of acupuncture at Shenshu acupoint on pain was better than that of non-meridian and non-acupoint group.Conclusion: 1.After the establishment of the rat model,the threshold of contraction of the claws stimulated by heat was significantly lower than that of the normal rats,and there was an allergic reaction to pain.Therefore,the rat model of non-compressive LDH nucleus pulposus protrusion was successfully established.2.The nerve conduction velocity of lumbar intervertebral disc herniation rat model treated by acupuncture at Shenshu point has been improved obviously,which indicates that acupuncture at Shenshu point can improve nerve conduction function.3.The content of substance P in the spinal cord and hypothalamus of the rat model was significantly decreased after acupuncture at Shenshu point,indicating that acupuncture at Shenshu point has certain analgesic and anti-inflammatory effects.4.The morphological changes of L5 nerve roots in each group before and after treatment were observed by light microscope.The morphological changes of L5 nerve roots after treatment had obvious repair effect compared with those before treatment.Compared with model group and non-meridian and non-acupoint group,the morphological changes of L5 nerve roots also had obvious promotion effect on nerve root repair.5.Through the analysis of the curative effect of acupuncture,the therapeutic effect of Shenshu group is obviously better than that of non-meridian and non-acupoint group,which shows that acupuncture at Shenshu has better therapeutic effect on lumbar disc herniation and further proves the specificity of meridian points.
