Professor Jiang Shumin's Academic Thoughts On The Treatment Of Diarrhea-type Irritable Bowel Syndrome And Observation Of Clinical Efficacy

This paper is mainly composed of Professor Jiang Shumin's academic thoughts on the treatment of DIARRHEA-TYPE irritable bowel syndrome and clinical efficacy analysis,in order to summarize Professor Jiang's academic thoughts and clinical experience in the treatment of IBS-D.Thesis I Research on the academic thought of Professor Jiang Shumin in the treatment of diarrhea type irritable bowel syndromeObjective: To explore Professor Jiang Shumin's academic thought and clinical medication analysis in the treatment of DIARRHEA-TYPE irritable bowel syndrome.Material and method:The outpatient medical records of patients with diarrheal irritable bowel syndrome who visited the outpatient clinic of Jiangshmin were collected by combining outpatient medical records study with Professor Jiangshmin's oral instruction.Professor Jiangshmin's experience on comprehensive clinical consulting and treatment of diarrheal irritable bowel syndrome was recorded with his teacher during clinical treatment,and the papers published by Professor Jiangshmin were consulted.Etc.,to explore Professor Jiang Shumin's theory and clinical experience of this disease.Conclusion:1.Professor Jiang Shumin believes that the pathogenesis of IBS-D is mostly related to the feeling of exogenous pathogens,emotional disorders,dietary injury,insufficient body mass and other factors,and its main pathogenesis is spleen deficiency and liver depression and dampness.2.In the treatment,Professor Jiang Shumin put forward the treatment ideas of strengthening and protecting the spleen and stomach,soothing the liver and relieving depression,eliminating dampness and removing turbidity,adding or subtracting the clinical symptoms according to the different symptoms,and cooperating with emotional intake,dietary control,etc.,with remarkable curative effect.3.In clinical differentiation,the disease is divided into five types: liver depression and spleen deficiency,spleen deficiency and dampness,spleen and kidney yang deficiency,spleen and stomach dampness and heat,cold and heat miscellaneous.4.Most prescriptions are made with peaceful medicines,which are added or subtracted according to the patient's condition,based on peaceful medicines and depicted slowly.Thesis II clinical researchObjective: In this study,110 patients with IBS-D of liver depression and spleen deficiency type were taken as the observation objects.The clinical effects of Jianpi Zhixie granule(prepared by Professor Jiang Shumin)and pinaverium bromide tablet(a Western Medicine)were compared in the treatment of IBS-D,and the difference between the two in the treatment of IBS-D was clarified.The effectiveness and safety of Shugan Jianpi Zhixie granule were observed.Materials and method: From October 2017 to October 2019,110 IBS-D patients with syndrome of liver depression and spleen deficiency who met the inclusion criteria of this trial were collected from Professor Jiang Shumin's outpatient department.According to the method of random numerical table,the patients in the group were divided into two groups: the treatment group and the control group,including 56 cases in the treatment group and 54 cases in the control group.The patients in both groups were given the same life intervention to keep their mood comfortable,quit smoking and alcohol,improve their eating habits,etc.The treatment group was given Shugan Jianpi Zhixie granule orally,once a day,once in the morning and once in the evening.The patients in the control group took the western medicine pinaverium bromide tablets(50mg / tablet),one tablet three times a day,swallowed in the meal.The treatment period was 8 weeks.After 8 weeks of treatment,statistical analysis and comparison of the efficacy of the two groups.Result:1.Through the statistical analysis of spss26.0,the differences between the two groups in gender,age and before treatment were comparable.2.The total effective rate of IBS-SSS in the two groups was 92.9% in the treatment group and 74.1% in the control group after 4 weeks of treatment(P = 0.008,P < 0.01).(2)Intra group comparison: the IBS-SSS scores after 4 weeks of treatment and after 4weeks of drug withdrawal were significantly higher than those before treatment(P <0.01).3.The total effective rate of the two groups was 92.9% in the treatment group and66.7% in the control group,respectively(P < 0.01).(2)Intra group comparison: after 4weeks of treatment and 4 weeks of drug withdrawal,P values were 0.000 and 0.000respectively(P < 0.01),and there was also significant statistical significance in the symptom score of the two groups4.Comparison between the two groups: after 4 weeks of treatment,there was a significant difference between the two groups(P < 0.01).(2)Intra group comparison:the scores of TCM symptoms in the two groups after treatment were significantly lower than those before treatment(P < 0.01).Conclusion: The treatment group(Shugan Jianpi Zhixie granule)and the control group(pinaverium bromide tablet)can effectively alleviate the common clinical symptoms of IBS-D patients,and the effect of the treatment group is more significant in the improvement of the total effective rate,the improvement of abdominal pain symptoms,the reduction of common symptoms of diarrhea and the reduction of recurrence rate.
