Purpose:To observe the clinical effect of acupoint selection guided by medical infrared thermography and conventional acupoint acupuncture in the treatment of acute scapulohumeral periarthritis,to explore the regularity of acupoint selection and the correlation between the degree of pain and the magnitude of temperature difference,so as to provide the basis for the visualization and regularity of acupoint selection in the treatment of scapulohumeral periarthritis.Material and method:72 patients with scapulohumeral periarthritis were collected from traditional Chinese medicine clinic and orthopedic clinic of Shengjing Hospital from September 2018 to March 2019.The patients who met the standard were randomly assigned to routine acupoint selection group(routine group)and infrared guided acupoint selection group(infrared group)according to the random number table method,36 cases in each group.The routine group was treated with acupuncture according to the main points in the ninth edition of acupuncture and moxibustion in the 12 th Five Year Plan textbook,which were Jianqian point,Jianyu point,Jianliao point,Jianzhen point,Quchi point and Yanglingquan point.Before the treatment,the infrared group took the infrared thermogram,and selected the commonly used acupoints(Jianqian point,Jianyu point,Jianliao point,Jianzhen point,Binao point,Naohui point,Jianjing point,Naoyu point,Quchi point and Tianzong point)around the shoulder to treat scapulohumeral periarthritis in the infrared thermogram were used to compare the temperature difference on the healthy side.The six points with the largest temperature difference were selected for acupuncture treatment.The two groups were treated once a day,once a week,and two weeks later,the treatment effect of the two groups was evaluated by comparing VAS score,shoulder joint activity and dash upper limb function questionnaire.The correlation between the degree of pain and the temperature difference was also analyzed.Use of measurement data SPSS20.0 Statistical software for analysis,detection level a=0.05.Results:1.There was no significant difference between the two groups in gender,age and course of disease(P > 0.05).2.There was no significant difference in VAS score between the two groups before treatment(P > 0.05).After treatment,the VAS score of the two groups decreased,and the effect of the infrared group was better than that of the routine group(P<0.05).3.There was no significant difference between the two groups in shoulder joint mobility and upper limb function of DASH before treatment(P > 0.05).After treatment,the two groups were improved,and the shoulder joint mobility and DASH upper limb function investigation score of infrared group were significantly better than that of routine group,the difference was statistically significant(P< 0.05).4.The difference of effective rate between the two groups was statistically significant,and the effective rate of infrared group was better than that of routine group(P<0.05).5.Under the guidance of infrared,the six acupoints with the highest frequency were Jianyu,Jianliao,Quchi,Naohui,Binao and Naoyu.6.VAS score was positively correlated with average temperature difference on the healthy side,and the greater the temperature difference,the greater the pain.Spearman correlation coefficient significance test results showed that r values were all P < 0.01.Conclusion:1.Infrared guided acupoint selection acupuncture can effectively relieve pain,improve shoulder joint function and improve shoulder joint mobility in the treatment of scapulohumeral periarthritis.2.The effect of infrared guiding acupuncture is better than that of routine acupuncture.3.The selection of acupoints guided by infrared thermography can be used as a new way of acupuncture for scapulohumeral periarthritis.
