Investigation And Analysis Of Influencing Factors Of Nurses' Knowledge And Practice In The Prevention Of Deep Vein Thrombosis During Operation

Purpose:To construct a survey questionnaire on the status of KAP(Knowledge-Attitude-Practice)of nurses in the prevention of DVT(deep vein thrombosis)during operation,and to investigate the status of the KAP of prevention and treatment of deep vein thrombosis in nurses in the operating room using questionnaires and to explore its related factors.Material and method:1 Construction of questionnaire Through literature research and expert meeting,the investigation team preliminarily determined the survey indicators of KAP of nurses for prevention of DVT during operation,and constructed relevant questionnaires.Delphi method was used to carry out two rounds of Experts enquiry for index selection,and the importance of each indicator of the questionnaire was assigned in the form of questionnaire,and the familiarity and judgment basis were scored.In September 2019,70 nurses in the operating room of Grade-A Tertiary hospitals were selected for a pre investigation to analyze the reliability and validity of the questionnaire.2 The status investigation of the KAP Between November and December of 2019,the convenient sampling method was used to extract 13Grade-A Tertiary Hospitals in 7 cities of a certain province.285 nurses were investigated by questionnaire,273 of which were included in the effective questionnaire for statistical analysis.At the same time,t-test,one-way ANOVA or non-parametric test,multiple linear regression analysis were used to analyze the influencing factors,and Pearson correlation analysis was used to analyze the relationship between the dimensions of the questionnaire.Result:1 The result of Delphi method and pre investigation(1)Expert reliability: During the Delphi correspondence process,the positive coefficients of the experts in both rounds were 100%,the authority coefficients Cr were 0.80 and 0.83,respectively,the coordination index W value was higher than the secondary index,and thesecond round of indicators W values are higher than the first round(P <0.05);After two rounds of expert enquiry by Delphi method,a questionnaire for the status of the nurses' KAP of deep vein thrombosis prevention with 5 primary indicators and 32 secondary indicators was finally established.(2)Reliability and validity of the questionnaire: According to the preliminary investigation and analysis,the total Cronbach's coefficient of the questionnaire constructed in this study was 0.865,and the Cronbach's coefficient of all indicators was greater than 0.7.The content validity value of questionnaire entry level(I-CVI)and the content validity value of questionnaire level(S-CVI)were both greater than 0.800.The overall KMO value of the questionnaire was 0.824,and the Bartlett sphericity ?2 value was2211.650(P < 0.05).Principal component analysis was used to extract 5 common factors with initial characteristic values greater than 1,and the variance characteristic values of the common factors were all greater than 0.4,and the cumulative variance contribution rate was80.56%?2 The results of status investigation(1)Scores of the questionnaire: 273 nurses in the operating room had a total score of(73.79±4.65)of the current status of KAP to prevent Deep vein thrombosis.Among them,the average score of "the department management status quo" is 1.23,and the average score of "the operating room nurse training status quo" is 1.56,the average score of "the operating room nurses cognitive status quo" is 3.45,the average score of "the operating room nurses attitude status quo" is(41.30±3.92),and the average score of "the operating room nurses practice status quo " is(26.24±1.95).(2)Univariate and Multivariate analysis results: the results show that the final education of nursing staff,qualifications of specialized nurses,job titles of nurses,and hospital area are influential factors that affect the scores of department management status quo;qualifications of specialized nurses are influential factors that influence the scores of training status quo;working years in operating rooms,qualifications of specialist nurses,job titles are influencing factors that affect the scores of cognitive status quo;initial qualifications,final qualifications,and qualifications of specialist nurses are the influencing factors that influence the scores of status quo of attitude;final qualifications,working years in the operating room,and the qualifications of specialist nurses are the influencing factors that influence the scores of practice status quo.(3)Correlation analysis:The correlation analysis results of various indicators show that was a positive correlation between management status and attitude status,behavior status,between training status and cognitive status,between cognitive status and attitude status,behavior status,between attitude status and behavior status(r= 0.147-0.288,P <0.05).Conclusion:1.The survey scale constructed in this study can better reflect the status quo of KAP of prevention and treatment of DVT during the operation of nursing staff in the operating room;2.At present,the overall status of KAP of deep vein thrombosis prevention in the operating room nurses of Grade-A tertiary hospitals in a certain Province is not very ideal.The mean scores of the status quo of department management,training,cognitive and practice are below average,but the mean score of status quo of attitude is above average.The department management,nurse training,nurse cognitive level and nursing behavior level related to the prevention of DVT need to be further improved;3.Nursing staff's final education,specialist nurse qualifications,operating room working years are the main factors affecting the status quo of KAP of operating room nursing staff in the prevention of DVT.It can be considered to enhance the level of KAP by strengthening management training,improving the educational level of nurses and increasing the number of specialized nurses.
