Background:Ulcerative colitis is one of the main inflammatory bowel diseases,which is characterized by intestinal ulcer and chronic inflammation.It has a long course,is easy to relapse,lingering and hard to heal.It is a difficult disease in the digestive system.When various pathogenic factors jointly cause the IL-23/Th17 cell inflammation axis to dysfunction,under the action of IL-6,the expression of Th17 cells and its characteristic cytokine IL-17 increases,causing an inflammatory response and eventually leading to the pathogenesis of UC.At present,modern medicine has no radical cure method for it,so it is necessary to find a new and more effective treatment method.Traditional Chinese medicine has certain advantages and characteristics in the treatment of UC,and the method of clearing heat,eliminating dampness and strengthening spleen has good clinical effect on UC,which can effectively alleviate symptoms and reduce recurrence.Objective:The pathogenesis of ulcerative colitis is related to the abnormal immune response of intestine.The the immunity of IL-23/Th17 plays an important role in the genesis,development and prognosis of UC.This paper aims to investigate the effect of clearing heat,eliminating dampness and strengthening spleen on the immunity of IL-23/Th17 and explore the mechanism of clearing heat,eliminating dampness and strengthening spleen on UC.Method:SPF male rats were randomly divided into four groups:normal group,model group,traditional Chinese medicine group,and western medicine group.The normal group was not allowed to make the model.The model group,Chinese medicine group,and western medicine group all used TNBs/anhydrous ethanol method to prepare the UC model.After the successful modeling,the Chinese medicine group and the western medicine group were given clearing heat,eliminating dampness and strengthening spleen decoction and Mesalazine,respectively 7 days of treatment.Serum and colon tissue specimens were taken from each group of rats,and the mucosal injury index score was used to evaluate the intestinal mucosal damage of the rats.The pathological changes of the colon tissue of the rats were observed under the microscope by HE staining,and the IL-6,the levels of IL-23 and IL-17 were detected by immunohistochemistry in colon tissues.Result:1.The rats in the model group had poor activity and eating conditions.They had mucus pus and blood,and the stool did not form.The Chinese medicine group and western medicine group had significantly improved activities and eating,and the mucus pus and blood decreased.2.The mucosal injury index scores of the model group rats were higher than the normal group(P<0.05),and the mucosal injury index scores of the Chinese medicine group and western medicine group were significantly lower than the model group(P<0.05);3.The colon tissue of rats in the model group has obvious inflammation and ulcerative lesions.The degree of inflammation and ulcerative lesions in the colon of rats in the traditional Chinese medicine group and western medicine group is significantly better than that of the model group;4.The IL-6 content in the serum of the model group rats was higher than the normal group(P<0.05),and the IL-6 content in the serum of the Chinese medicine group and the western medicine group was significantly lower than the model group(P<0.05);5.The IL-23 content in the serum of the model group rats was higher than that in the normal group(P<0.05),and the IL-23 content in the serum of the rats in the Chinese medicine group and the western medicine group was significantly lower than that in the model group(P<0.05);6.The IL-17 content in the serum of the model group rats was higher than that in the normal group(P<0.05),and the IL-17 content in the serum of the Chinese medicine group and the western medicine group was significantly lower than that in the model group(P<0.05);7.The expression of IL-6 in the colon tissue of the rats in the model group was significantly higher than that in the normal group,and the expression of IL-6 in the colon tissue of the rats in the Chinese medicine group and the western medicine group was significantly less than that in the model group;8.The expression of IL-17 in the colon tissue of the model group was significantly more than that in the normal group,and the expression of IL-17 in the colon tissue of the Chinese medicine group and western medicine group was significantly less than that in the model group.Conclusion:One of the mechanisms of clearing heat,eliminating dampness and strengthening spleen in treating UC may be to reduce the expression of IL-23,IL-6,inhibit the differentiation of Th17 cells,reduce the secretion of IL-17,reduce the inflammation induced by the IL-23/Th17 cell axis and Damage to the intestinal mucosa.
