| BackgroundChronic gastritis(CG)is a chronic inflammation of gastric mucosa caused by many factors,which is a common and frequently-occurring disease of digestive system.It is usually clinically classified as chronic atrophic gastritis(CAG)and chronic non-atrophic gastritis(CNAG)based on endoscopic findings.As a common digestive system disease,CG is prevalent around the world,and its incidence is increasing annually.Therefore,the treatment of CG should arouse more attention of the medical profession.Recently,Chinese medicine has played an important role in the treatment of CG.Compared with western medicine,traditional Chinese medicine(TCM)has advantages of significant effects,lower side effects and reducing recurrence in treating CG.However,the treatment of TCM in this field is mainly based on experience summary and analysis yet.As opinions vary,there isn't enough persuasion and instruction significance.Therefore,scientific research of TCM is of great value in the treatment of CGConstitution is closely related to the occurrence and development of many diseases.The constitution theory of TCM plays an important role in many aspects,such as physiology and pathology,disease occurrence,diagnosis and treatment,prognosis,prevention and regulating Therefore,exploring the distribution of constitution type can identify the susceptible population of the disease,and it is valuable for understanding the basic pathogenesis of the disease,treatment and medication at the same time.ObjectiveChinese medicine plays an important role in the treatment of CG.This study intends to use data mining technology to analyze the TCM prescriptions for CG patients treated by Professor Zhang,discuss the medication rules,provide scientific basis for summarizing and learning Professor Zhang's experience of treatment and prescription ideas,and provide some valuable reference for the treatment of the disease.The distribution of constitution types of CG and the risk factors of CAG were explored and analyzed in this study.And the correlation between them and the occurrence and development of CG was discussed,so as to provide basis for the diagnosis,treatment,prevention and regulating of CG in TCM.Methods1.Case collection and data analysisIn this study,the 200 CG medical records treated by Professor Zhang in the outpatient department of Dongfang Hospital from January 2018 to January 2020 were collected.The database containing patients' general information and treatment medication was established by Microsoft Excel 2016 and analyzed by IBM SPSS Statics 20.0 and IBM SPSS Modeler 18.0.2.Questionnaire distribution and statistical analysisThe personal information,general situation and constitution type of 377 patients with CG were investigated by questionnaire.The database was established by Microsoft Excel 2016 and analyzed by IBM SPSS Statics 20.0.3.Correlation studyIn contrast with the distribution of constitution types of CG patients and the treatment experience of Professor Zhang,descriptive analysis was carried out to preliminarily discuss the correlation between them.Results1.The results of the study on the medication experience in treating CGThe 200 patients consist of 81 males and 119 females.The average age was 52.26 ±12.04 years,and the age of CAG was higher than that of CNAG.There was no significant difference in symptom spectrum between CNAG and CAG.According to the statistics of the frequency of Professor Zhang's medication,154 traditional Chinese medicines were obtained.Statistics of high-frequency drugs shows that the common drugs in the treatment of CG included qi-regulating drugs,heat-clearing drugs,cough and asthma-relieving and phlegm-removing drugs,blood activating drugs,water-promoting and dampnes-removing drugs,digestant drugs,tranquilizing drugs,dampness-removing drugs,interior-warming drugs,hemostatic drugs,liver and wind-calming drugs,tonic drugs and astringent drugs.The medical properties of the drugs used more warm,cold,mild and less heat and cold.As for the drug taste seletion,bitter,pungent and sweet drugs were used mainly,while salty and sour drugs were used less.The selected drugs were mainly acting on spleen,stomach,lung,liver meridianThrough clustering analysis and correlation analysis of CNAG and CAG,the basic drugs,addition and subtraction drugs,commonly used drugs,12 drug pairs and 5 combinations of common drugs were obtained.2.Investigation on the distribution of constitution types of CGThe 337 CG patients consist of 153 males and 224 females.The average age was 50.77±13.78 years,and the age of CAG was higher than that of CNAG.The average body mass index(BMI)was 23.65 ± 2.93,and 90.5%of the BMI was normal or overweight.The proportion of CAG patients infected with Hp was higher.There was no significant difference between CNAG and CAG in family history of gastric cancer,smoking and drinking history and mental state.The distribution of constitution types of CG:mild physical constitution 45.6%>Yang-sufficiency constitution 18.0%>dampness-heat constitution 17.5%>Yin-sufficiency constitution 14.3%>Qi-stagnation constitution 12.5%=phlegm-dampness constitusion 12.5%>specific endowment constitution 7.2%>Qi-sufficiency constitution 5.8%>blood-stagnant constitution 3.7%.The distribution of Yin-deficiency constitution and dampness-heat constitution in CNAG and CAG was different.There were more dampness-heat constitution in CNAG patients and more Yin-deficiency constitution in CAG patients.The distribution of other constitutions was basically the same between the two groups.3.Correlation studyBoth studies identified age as an important factor in the pathogenesis of CAG,it showed that the incidence of CAG increased with age.Yin-deficiency constitution accounts for a higher proportion in CAG patients,while dampness-heat constitution accounts for a higher proportion in CNAG patients,which is related to the evolution of the disease,and also reflected in clinical medicationConclusion1.Age and Hp infection are the main risk factors of CAG.2.There is a difference between CNAG and CAG in the distribution of constitution types,which has certain guiding significance for clinical medication.The distribution of Yin-deficiency constitution and dampness-heat constitution in CNAG and CAG was different There were more dampness-heat constitution in CNAG patients and more Yin-deficiency constitution in CAG patients3.Through clustering analysis of high-frequency drugs,it shows that Xiangsu powder and Erchen decoction are the main prescriptions in the treatment of CNAG.Xiangsu powder,Danshen decoction and Baihe Wuyao decoction are the main prescriptions in the treatment of CAG.4.In the aspect of experience summary,this study summarizes Professor Zhang's understanding of the core pathogenesis and disease location of chronic gastritis,as well as the basic principles of diagnosis and treatment,so as to provide some reference for clinical treatment of chronic gastritis. |
