Object1.Through the investigation of the risk factors and the changes of the patients’ condition before the onset of acute stroke,the risk factors that cause the occurrence of stroke are summarized,and the clinical features with predictive value before the occurrence of stroke are explored,so as to provide early warning signals for the prevention of stroke.2.Through the study of the clinical characteristics of stroke high-risk patients and stroke patients’ disease changes,to explore the evolution of stroke from high-risk state to stroke occurrence,so as to provide theoretical basis for stroke prevention of high-risk patients.MethodsFrom October 2010 to February 2020,591 patients with acute ischemic stroke who were hospitalized in Dongzhimen Hospital,Dongfang Hospital,Fangshan District Hospital of traditional Chinese medicine,Beijing Hospital of traditional Chinese medicine,Taiyuan Central Hospital,Taiyuan Hospital of traditional Chinese medicine,Taiyuan Hospital of integrated traditional Chinese and Western medicine outpatient and ward were investigated retrospectively as stroke group.And 108 high-risk stroke patients from March 2018 to February 2020 were prospectively investigated,and the occurrence of stroke end-point events was recorded 3 months later.The patients without stroke end-point events were regarded as high-risk groups.Using a unified questionnaire,the demographic data(gender,age),basic medical history(hypertension,diabetes,hyperlipidemia,coronary heart disease,atrial fibrillation and other medical history)and control conditions,four diagnostic information of disease changes(including 58 items such as consciousness,emotion,head and facial features,visceral functions,transient symptoms,etc.),the personal history(body type,personality,work nature,smoking and drinking habits,food taste,exercise)and family history of cardiovascular and cerebrovascular diseases of the two groups were collected and recorded.In addition to the clinical information collected from the above two groups,the acute stroke patients also included the incidence of stroke(first episode,recurrence,inducement,control of related basic diseases before the onset,onset state,season,onset main syndrome,admission diagnosis of traditional Chinese and Western medicine,TCM syndrome and toast classification).According to the collected data,the relevant database was established,and spss22.0 statistical software was used for descriptive statistics,chi square test and logistic regression analysis.Results1.Study on the clinical characteristics of the changes of stroke patients before onset.(1)A total of 591 patients with acute cerebral infarction were included,391 of them were male and 200 of them were female.544 patients(92.0%)had disease changes,and 47 patients(8.0%)had no disease changes.Stroke can occur in all seasons of the year.Summer and autumn is the season with high incidence of stroke.53.5%of stroke patients have inducements before onset,especially fatigue,anger or rage.The inducements in summer and autumn are more prominent than that in winter and spring,especially in weather change.In the acute stage of stroke,the syndrome of wind phlegm stasis and wind fire disturbance are the top two syndromes.84.4%of stroke patients have risk factors,311 of them have more than 2 risk factors,accounting for 52.6%;280 of them have less than 2 risk factors,accounting for 47.4%.The distribution of risk factors from high to low is:hypertension,diabetes,hyperlipidemia,coronary heart disease,and patients with multiple risk aggregation are more likely to relapse stroke.(2)Before the occurrence of stroke patients with different risk levels,the changes of their condition showed the syndrome characteristics of "wind","fire","phlegm","blood stasis" and"Qi Deficiency",while the patients with multiple risk factors have more obvious symptoms,such as no rising of Yang,no nourishing of orifices and internal wind.(3)The analysis of the clinical characteristics of stroke patients in different ages before onset suggests that the elderly patients have more obvious manifestations such as clearing the orifices and losing nourishment,hyperactivity of liver Yang and dysfunction of viscera Qi mechanism.(4)The analysis of clinical characteristics of stroke patients without disease change shows that the average age of such patients is relatively small,mainly male.The season of onset is mainly in summer.Considering the inducement is related to fatigue and weather change,most of them are first-time patients,with fewer risk factors,more irritable personality,less smoking and drinking,and less family history.2.A comparative study on the clinical characteristics of stroke high risk patients and stroke patients.(1)Comparing the clinical characteristics of stroke patients and high-risk patients in the same period,the high-risk stroke patients should be alert to the occurrence of stroke when they have the changes of consciousness emotion and transient symptoms of "clearing the orifices and losing nourishment" and "the image of internal wind".And patients with multiple risk factors have more obvious "wind" and "fire" before stroke.(2)The proportion of somnolence and paroxysmal hemiplegia in stroke group was significantly higher than that in high risk group(P<0.05).Conclusion1.The incidence rate of stroke patients increased gradually in summer and autumn.The occurrence of stroke was mostly caused by multiple risk factors,and the results of inducements were also induced.Therefore,stroke patients should avoid inducing factors,control risk factors actively,and prevent stroke in summer.2.There are traces of stroke.The changes of the disease condition before the occurrence of stroke conform to the pathogenesis theory of "toxic damage to brain and collaterals",and the syndrome of "wind" and "fire" plays a leading role in the occurrence of stroke.It is a dynamic evolution process from patients with multiple risk factors to the occurrence of stroke.When there are mental emotions and transient changes,it is necessary to be aware of the occurrence of stroke.3.Patients without disease change should pay attention to the inducement,pay attention to the law of daily life,and be alert to the occurrence of summer stroke.
