Improving the use of preventive health care in acute care settings to decrease rates of disease in individuals with heightened risk factors

Abstract/Summary:
The issue of low utilization rates of preventive health care programs was researched in this capstone. The purpose of this research was to develop a hypothetical program that may be implemented in a healthcare setting to increase the use of preventive health programs. The target demographic was individuals who were at a higher risk of developing chronic illnesses and may benefit from preventive health programs. The method used to develop this hypothetical program was to study the "champion" model at Lenox Hill hospital, the program that was used to successfully change the culture of the facility and establish desired change. It was discovered that based on the success of the Fall Prevention champion and Catheter Associated Urinary Tract Infection (CAUTI) champion models, a preventive healthcare champion was a successful way to increase the utilization of preventive health programs.
