Abstract/Summary:
Hospitals are concerned with 30 day readmission rates because it affects their reimbursement and are publicly reported information. The most difficult part about 30 day readmission rates is, for hospitals in urban communities with multiple hospitals in the local area, being able to track the readmitted patients that go to other hospitals. The business plan is to contract with 16 hospitals in an identified community and have them submit all patient data to a data analytic company to identify trends and readmission rates of ancillary facilities and home patients. This will provide a detailed report card on each facility so that hospitals can identify where there patients are being treated best and which facilities have higher readmission rates.
