Abstract/Summary:
Background and ObjectionCapsule endoscopy has been developed for more than 20 years.In traditional sense,capsule endoscopy has been regarded as an important means to evaluate the diagnosis and severity of Crohn’s disease,and it also brings definite diagnostic and therapeutic value for unexplained gastrointestinal bleeding.However,nowadays,capsule endoscopy is mainly used for the examination of small intestinal diseases.Many new capsule endoscopy techniques at home and abroad extend to the examination of esophagus,stomach,colon and other parts.Among them,specific parts are mainly examined,and a few new capsule endoscopes can complete the whole digestive tract examination.In recent years,capsule endoscopy system controlled by magnetic force is more popular.Among them,the research and development of Anhan’s "Magnetically Controlled Capsule Gastroscopy" Robot and its clinical application license are well-known all over the world.The capsule endoscopy system produced by Anhan has become the world’s first solution to the "immobility" of capsule endoscopy through the combination of magnetic force control and capsule endoscopy system.The difficult problem also breaks the myth that capsule endoscopy mainly uses small intestine examination.Nevertheless,the magnetically controlled capsule gastroscopy system is different from the traditional capsule endoscopy.The weight of the capsule,the purchase cost of the capsule and the system equipment,the technical requirements for the inspectors,the requirements for the site size of the magnetically controlled equipment and the requirements for the implementation of the loop endoscopy are obviously different.These viewpoints may become the influencing factors for the practical application and popularization of the technology in clinical practice.In order to enable traditional capsule endoscopy to obtain more gastric examination ability without changing its technical parameters,this study attempts and assumes that active body position conversion is used instead of magnetic force to control the gastric movement of traditional capsule endoscopy and complete a series of gastric examinations.MethodsDuring the period from September 2018 to December 2018,40 subjects were recruited from the Center of Gastroenterology-National Capsule Endoscopy Research,Southern Hospital of Southern Medical University.They were randomly divided into two groups:the capsule endoscopy gastroscopy group(experimental group)with active body position conversion and the capsule endoscopy group(control group).Subjects must be included in the following inclusion criteria:1.Age 18 to 65 years;2.No dysphagia;3.No previous surgical history;4.No digestive endoscopy was performed in the past year;5.No drugs were used in the past three months;6.defecation at least once a day,no more than two times.In addition,subjects could not meet any of the following exclusion criteria:1.Pregnant women or those over 65 years of age;2.allergic to polyethylene glycol or siloxane oil;3.patients with history of gastrointestinal obstruction such as pyloric obstruction and intestinal obstruction;4.patients with history of gastrointestinal surgery;5.patients with chronic constipation or diarrhea;6.patients with other serious primary diseases of heart,cerebrovascular,liver,kidney and hematopoietic system;7.The gastrointestinal tract is congenitally malformed or abnormal.ResultsCompared with the active position conversion stomach capsule endoscopy examination group(experimental group)and the position of the autonomous capsule endoscopy group(control group),the experimental group full coverage of the structure of gastric was 94.4%,The coverage of gastric fundus and cardia was 94.74%and 78.95%respectively.The coverage of gastric body,antrum,antrum and pylorus was 100%.There were no significant difference between the two groups in stomach passage time,small intestine passage time and colon passage time(P>0.05).ConclusionThe traditional small intestine capsule endoscope can effectively cover the main anatomy of the stomach through active body position conversion.This method does not affect the passage time of the lower digestive tract during the examination of the stomach.It is safe and reliable to change the stomach of the body group.No serious adverse events such as capsule endoscopy retention have been found.
