Comparative Study Of TAPP And Open Preperitoneal Space Hernia Repair In The Treatment Of Elderly Inguinal Hernia

Abstract/Summary:
Objactive: To investigate the clinical effect of TAPP and Open preperitoneal space hernia repair in the treatment of primary unilateral inguinal hernia in the elderly.Method:The clinical data of 61 elderly inguinal hernia patients who received TAPP and open preperitoneal space hernia repair in the general surgery department of the Sixth Affiliated Hospital of Xinjiang Medical University from December 2019 to November 2020 were analyzed.30 cases were in the treatment group and received transabdominal preperitoneal hernia repair(TAPP),31 cases were in the control group and received open preperitoneal space hernia repair.The surgical effects of the two surgical procedures were compared and the incidence of postoperative complications and the VAS pain score within 3 months after the operation were compared between the two groups.Result: There was no significant difference in general data between the two groups(P>0.05);There were statistically significant differences between the treatment group and the control group in terms of operation time,intraoperative blood loss,postoperative time to get out of bed,postoperative hospital stay,and total hospitalization expenses(P<0.05).Complications occurred in 2 cases(6.7%)in the treatment group,and 10 cases(32.3%)in the control group,the difference was statistically significant(P<0.05).However,there was no significant difference in postoperative complications such as incision infection,scrotal effusion,incision seroma,foreign body sensation in the operation area,and hernia recurrence between the two groups of patients,and the difference was not statistically significant(P>0.05).There was a statistically significant difference in VAS pain scores between the treatment group and the control group at 1 day after surgery,1 week after surgery,and 1 month after surgery(P<0.05),but there was no significant difference in VAS pain scores at 3 months after surgery(P>0.05).Conclusion: Both TAPP and open preperitoneal space hernia repair can be applied in the surgical treatment of patients with primary unilateral inguinal hernia.Compared with the two surgical methods,TAPP has the advantages of relatively low intraoperative blood loss,early postoperative getting out of bed,short postoperative hospital stay,light postoperative pain,and fewer postoperative complications.However,TAPP requires the establishment of intra-abdominal pneumoperitoneum under general anesthesia,which has higher requirements on the overall condition of the patient,and the operation time is relatively long and the operation cost is relatively high.But the open preperitoneal space hernia repair needs less operation time,total hospitalization expenses are low.There is no significant difference between the two surgical methods for postoperative wound infection and hernia recurrence.
