PurposeDiabetic retinopathy is the most common microvascular complication in patients with type 2 diabetes,and its pathogenesis is complex.Previous studies have shown that inflammatory response is closely related to its occurrence and development.As an emerging indicator of inflammation,Monocyte lymphocyte ratio MLR has been proven to be correlated with the prognosis of various diseases,but its correlation with diabetic retinopathy has been rarely studied.The purpose of this study was to investigate the correlation between MLR and type 2diabetic retinopathy,so as to detect and treat the disease early.MethodA retrospective study was conducted on 300 patients with type 2diabetes who were clearly diagnosed in China-japan Friendship Hospital,Jilin University from June 2019 to June 2020.General information and clinical biochemical indexes of the patients were collected.Including gender,age,height,weight,duration of diabetes,history of high blood pressure,fasting plasma glucose(FPG),glycosylated hemoglobin(Hb A1c),white blood cell count(WBC),neutrophil count(Nc),lymphocyte count(Lc)and mononuclear cell count(Mc),hemoglobin(HGB),platelet count(PLT),blood urea nitrogen(SBUN),serum creatinine(SCr),blood uric acid(SUA),total cholesterol(TC),triglycerides(TG),high-density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),and other indicators;NLR=neutrophil count/lymphocyte count,PLR= platelet count/lymphocyte count,MLR= monocyte count/lymphocyte count were calculated.SPSS22.0 statistical software was used to process and analyze the data.Results1.The differences in age,duration of diabetes,history of hypertension,SUA,BUN,Cr,WBC,Nc,Mc,HGB,NLR and MLR between DR group and NDR group were statistically significant;The WBC,Nc,Mc,HGB,NLR and MLR were not statistically significant in NPDR group and PDR group.2.All patients were grouped according to the MLR quartile spacing level(Q1-Q4),and divided into Q1 group(MLR<0.1720),Q2 group(0.1720<MLR <0.2129),Q3 group(0.2129<MLR<0.2630),Q4 group(MLR>0.2630)to conduct a trend test.The results showed that the prevalence of DR increased with the increase of MLR quartile(21.25%,23.13%,25.00%,30.63%,respectively),and the difference was statistically significant.3.Logistic stepwise regression analysis of general data,biochemical indexes and inflammatory indexes of T2 DM patients and DR showed that age,course of diabetes,history of hypertension,decrease of BUN,SUA and HGB were independent risk factors for DR.Conclusion1.The ratio of monocytes to lymphocytes is related to the occurrence of diabetic retinopathy in patients with T2 DM,and the risk of diabetic retinopathy increases with the increase of the ratio of monocytes to lymphocytes..2.Age,course of diabetes,history of hypertension,elevated levels of blood urea nitrogen and uric acid,and decreased hemoglobin in patients with type 2 diabetes are independent risk factors for diabetic retinopathy.
