Objective: To compare the clinical efficacy and safety of cortical bone screw(CBT)and pedicle screw(PS).Method: Retrospective analysis was performed on 66 patients with lumbar degenerative diseases complicated with osteoporosis who underwent surgical treatment in the Department of Spinal Surgery of Wannan Medical University from July 2018 to September 2019.Among them,34 patients were treated with CBT screw and 32 patients were treated with PS screw.The gender,age,operation time,intraoperative blood loss,postoperative drainage volume,postoperative hospital stay,complications and other conditions of the patients were completely recorded.The Visual Analogue Pain Scale(VAS),the Oswestry Disability Index(ODI),and the Japanese Orthopaedic Association Assessment Treatment Score(JOA)were used to evaluate function before and after surgery and at the last follow-up.Paired t test was used for intra-group data comparison,two independent samples t test was used for inter-group comparison,and X2 test was used for counting data comparison.Results: All patients were followed up for at least 12 months,and there were no postoperative complications such as loosening,nail withdrawal or nail fracture in both groups.The operative time,intraoperative blood loss,postoperative drainage volume and postoperative hospitalization days in the CBT group were significantly lower than those in the PS group,and the differences were statistically significant(t=-6.99,-8.64,-6.15,-10.66,all P=0.00).During the follow-up,VAS,ODI and JOA scores in the CBT group were all improved better than those in the PS group,and the differences were statistically significant(t=-6.71,-4.48,3.88,all P=0.00).Conclusion: CBT screw technique has the characteristics of minimally invasive,short operation time,less intraoperative bleeding,short postoperative hospital stay,quick postoperative recovery and so on.It is a good choice for patients with lumbar degenerative diseases associated with osteoporosis.
