Objective: To investigate the influence of T1 inclination angle on the sagittal parameters of cervical multi-stage myelopathy and posterior longitudinal ligament ossification before and after single door surgery. Methods: 58 patients with cervical Spondylotic myelopathy or ossification of cervical posterior longitudinal ligament were selected from January 2018 to December 2019 in the first affiliated Hospital of Xinjiang Medical University.the preoperative T1 tilt angle was measured by routine lateral cervical X-ray.C0-C2 Cobb angle,C2-C7 Cobb angle and C2-C7 SVA were compared before and at the last follow-up,and the changes of parameters before and after operation were compared. Results: Preoperative T1 slope group comparison of preoperative radiological parameters found that the two groups of patients with multi-stage cervical spondylosis and cervical posterior longitudinal ligament ossification had larger preoperative cervical lordosis, which was statistically significant(p<0.05),but there was no significant difference between the two groups of multi-stage cervical spondylosis and cervical posterior longitudinal ligament ossification.Regardless of the inclination angle, the C2-7 Cobb angles of the patients in the CSM group and the OPLL group were decreased compared with preoperatively at the last follow-up,but the missing angle in the m CSM group was significantly greater than that in the OPLL group(P<0.05);The angle of inclination CO-2Cobb angle increased compared with that before operation, and the increase was more obvious in patients in m CSM group(P<0.001).Regardless of the size of the postoperative C2-7SVA,the inclination angle of the two groups of patients increased compared with that before the operation. The difference with the increase of the upper cervical lordosis angle is that the OPLL group increased more significantly than the m CSM group, and both the CSM group and the OPLL group Although patients with a higher T1 slope before surgery have more cervical lordosis, they also have more postoperative kyphosis(P<0.001),that is,the CO-2Cobb angle is larger, and the C2-7SVA and C2-7Cobb angles are larger.Small,T1 tilt angle C2-7SVA,C0-2Cobb angle are significantly positively correlated, and C2-7Cobb angle is negatively correlated. Conclusion: The larger the T1 inclination angle, the greater the impact on the sagittal parameters before and after the cervical single-door spinal canal enlargement, but the greater the impact on the postoperative cervical sagittal parameters in patients with multi-stage cervical spondylosis.T1 tilt angle C2-7SVA,C0-2Cobb angle are significantly positively correlated, and C2-7Cobb angle is negatively correlated...
