| ObjectiveA randomized controlled method was used to observe the short-term clinical efficacy of patients suffering from stage I and II blood stasis osteonecrosis of the femoral head(ONFH).To evaluate the short-term clinical efficacy of Gubi Huayu Decoction in patients with ONFH after core decompression of blood stasis type I and II,aiming to provide evidence for the promotion and application of Gubi Huayu Decoction in clinical practice.MethodsThe object of this study was a patient diagnosed with ONFH in the Department of Orthopedics,Sanming Hospital of Integrated Traditional Chinese and Western Medicine Affiliated to Fujian University of Traditional Chinese Medicine from October 2018 to September 2020.The study adopts the method of randomized controlled trials, and the research subjects are randomly divided into a control group and a test group.A total of 61 patients with ONFH who met the enrollment criteria were included in the study, and one case was dropped from the experimental group.30 cases in the control group received core decompression + prevention of infection(ceftazolin sodium),thrombosis(low molecular weight heparin sodium),protection of gastric mucosa(omeprazole)and postoperative pain relief(lornoxicam),the experimental group On the basis of the control group,30 cases were treated with "Gubi Huayu Decoction" for 3 months.By observing the visual scores of hip pain before and after treatment,Harris hip joint function scores, and quantitative scores of hip imaging effects, observe and record whether there are adverse reactions to evaluate the effectiveness of Gubi Huayu Decoction.Results1.A total of 61 patients were enrolled in the study,of which 1 case fell out(because the patient did not return to the hospital on time for follow-up,the patient was confirmed by telephone to be unwilling to continue to cooperate with the study),and the remaining 60 patients completed clinical observations.2.Before treatment, the baseline data(gender, age, height, weight, surgical hip,ARCO staging)of the two groups of patients were not statistically significant(P>0.05),and they were comparable. In addition, there was no statistically significant difference between the two groups of patients with hip VAS pain score,Harris hip joint function score, and quantitative imaging effect score before treatment(P>0.05).3.After treatment, the VAS pain scores of the two groups of patients decreased to varying degrees. There was a statistically significant difference in VAS scores between the two groups at 1,3,and 6 months after surgery(P<0.05).The test group was better than the control group;the Harris function scores of the two groups were improved before and after treatment. There were statistically significant differences in Harris scores between the two groups at 1 month,3 months, and 6 months after surgery(P<0.05),and the experimental group was better than the control group. There was no significant difference between the two groups of patients in the quantitative scores of imaging efficacy before and after treatment(P>0.05).4.After treatment, there were no obvious adverse reactions in the two groups of patients, and the changes in blood routine, liver and kidney function, coagulation function and other indicators before and after taking the medicine were all within the normal range.ConclusionGubi Huayu Decoction combined with core decompression has good short-term effects in the treatment of patients with stage Ⅰ and Ⅱ blood stasis ONFH,which can improve the patients’ hip pain symptoms and hip joint function, and delay the evolution of the disease to stage Ⅲ and Ⅳ. |
