The Effect Of Continuous Blocking Of Sciatic Nerve On Postoperative Analgesia In Patients With Foot And Ankle Injury

Abstract/Summary:
Objective:To compare the continuous sciatic nerve blockage that has emerged in postoperative analgesia for patients with foot and ankle injuries,and the most basic and most commonly used method of intravenous analgesia after surgery based on the concept of ERAS.And to explore the safer and the more effective method of them.As well as,evaluating the satisfactory of postoperative analgesia,and exploring the effect of continuous sciatic nerve blockage on postoperative analgesia in patients with foot and ankle injury.Method:60 patients with foot and ankle injury that requiring surgical treatment were divided into two groups according to the different methods of intraoperative and postoperative analgesia,they were distributed as 30 cases in each group: sciatic nerve blockage was the study group,in which the popliteal area of the postoperative patients was underwent continuous sciatic nerve catheterization blockage;the control group was treated with combined spinal-epidural anesthesia during the operation,and the postoperative patients were treated with intravenous analgesia.The postoperative analgesic effects of the two groups were compared(VAS scores at different time points after surgery,the earliest time of postoperative pain,and the number of additional analgesics after surgery);stress indicators(venous blood hypersensitivity C-reactive protein at half an hour after surgery),Blood glucose and interleukin-6(IL-6);systolic blood pressure and heart rate at different time points after surgery;early recovery(long stay in hospital,time to start foot and ankle joint activity after surgery,time to first walk in the ground after surgery);occurrence of adverse reactions and patient satisfaction degree.Result: VAS scores of the study group at 6h,12 h,24h,and 48 h after operation were lower than those of control group,P<0.05.The earliest postoperative pain time in the study group was 14.80 h,compared to 4.20 h in the control group,P<0.05.The number of additional tramadol in the study group was 0.73 times,compared to 1.77 times in the control group,P<0.05.The earliest postoperative pain in the study group was late,and the number of additional analgesic drugs was less.There was no significant difference in venous blood hypersensitivity C-reactive protein,blood glucose and IL-6 between the two groups after half an hour postoperation,P>0.05.There was no significant difference in systolic blood pressure and heart rate between the two groups postoperation,P>0.05.There was no statistically significant difference in the hospitalization time between the two groups,P>0.05.The time of first active ankle movement after operation and the first time of ground movement after operation were earlier in the study group than in the control group,the difference was significant,P<0.05.The postoperative adverse reaction rate was 63.3% in the control group compared to 16.7% in the study group,P<0.05.The patient satisfaction rate of the control group was 66.7% compared to 90% in the study group.The patient satisfaction of the study group was higher than that of the control group,P<0.05.Conclusion : Continuous sciatic nerve blockage can effectively alleviate the postoperative pain of patients with foot and ankle injury,which is beneficial for early recovery of patients.
