| Objective:To investigate the influence of the distribution of polymethylmethacrylate(PMMA)in osteoporotic vertebral compression fractures(OVCF)patient treated with percutaneous vertebroplasty(PVP),on the early postoperative therapeutic effect.Methods:A total of 132 patients,including 52 males and 80 females,with a mean age of(76.3±8.2)years who diagnosed as osteoporotic vertebral compression fractures in First Hospital of Shanxi Medical University from January 2017 to June 2019 were enrolled.All patients were divided into poor dispersion group(n=54)and d dispersion distribution group(n=78)according to the pattern of cement distribution on postoperative X-rays.Preoperative and postoperative visual analogue scores of patients in the dispersion distribution group and the poor dispersion group were compared respectively.The height of the anterior edge and midline of the injured vertebrae were measured,and the local kyphotic Cobb Angle was measured preoperatively and postoperatively for comparison.The incidence of postoperative complications was statistically analyzed.Results:(1)The VAS scores of patients in the dispersive distribution group and the poor dispersion group were lower than those before operation at 2 days,1 month and 6 months after operation(P < 0.05).The VAS score of the group with poor diffusion was higher than that of the group with diffuse distribution 1 month after operation(P < 0.05),and there was no significant difference between preoperative and postoperative 2 d and 6months(P > 0.05).(2)The local kyphotic Cobb Angle of the group with diffuse distribution and poor dispersion was recovered after operation(P < 0.05).The anterior edge and middle height of the vertebral body were also increased(P < 0.05).There was no statistically significant difference in the three groups between the disseminated group and the poorly disseminated group(P > 0.05).(3)There was no statistically significant difference in bone cement leakage between the two groups(P > 0.05).The incidence of refracture of adjacent vertebral body in the diffuse distribution group was lower than that in the diffuse poor group(P < 0.05).Conclusion:PVP is one of the effective treatments for OVCF.The distribution of bone cement in the injured vertebrae will affect the surgical effect,and the diffusion of bone cement distribution can reduce the recurrence of fracture in the adjacent vertebrae. |
