| Objective : Diabetes has become the third largest chronic non-communicable disease after tumors and cardiovascular diseases.Among them,the heritability,morbidity and mortality of type 2 diabetes are at a relatively high level,and the treatment cost is high and difficult to cure.Its prevention and control work has become a hot research issue in the field of international public health today.At present,the number of patients with type 2 diabetes is constantly increasing.The unreasonable increase in average hospitalization expenses and insufficient optimization of the cost structure are still the key tasks to be solved in medical cost control.These problems have caused huge economic pressure and heavy physiological burden on patients.The double burden of pain.Therefore,it is necessary to study the medical burden of patients with type 2 diabetes.This article analyzes the changes in the structure of hospitalization expenses for patients with type2 diabetes in N city from 2016 to 2019,and understands the main factors affecting the hospitalization expenses of patients,and provides a reference for reducing the economic burden of family diseases and curbing the unreasonable growth of medical expenses.Methods: Through time series analysis,this study took the information of hospitalization expenses of type 2 diabetes patients in the city during 4 years as the research object,and selected all patients discharged from hospital with type 2 diabetes(ICD-10 coded as E11)and their basic conditions and hospitalization expenses were counted.Description and analysis of results.The new gray correlation analysis is used to analyze the strengths and weaknesses of the proportions of hospitalization expenses such as drug fees,treatment fees,and inspection fees,and rank the relevance.The structure change analysis analyzes the structure change value and structure contribution rate of each proportion of hospitalization expenses.To further specify and analyze other indicators;use SPSS and AMOS7.0 software for further processing of relevant data,and at the same time use path analysis to analyze and study the influencing factors of hospitalization expenses,and establish and verify the model through structural equation model analysis.The influencing factors of the hospitalization expenses of diabetic patients were studied and analyzed separately.Results: From 2016 to 2019,the number of hospitalized patients with type 2diabetes in the city has been increasing year by year.The number of male patients is higher than that of females.The number of patients in the 60-79 age group is the largest,and the trend of younger patients is becoming more and more obvious.In2016-2018,hospitalization expenses showed an upward trend,but 2018-2019 showed a negative growth.The new gray correlation analysis shows that in the composition of hospitalization expenses for patients with type 2 diabetes,the top three ranks of the correlation are treatment fees,drug fees,and inspection fees.The structural change analysis shows that drug fees contribute the most to the structure of hospitalization expenses.Path analysis model results show that the effects of various factors on hospitalization costs in descending order are the number of days of stay,the proportion of drugs,age,tertiary hospitals,with or without complications,secondary hospitals,urban employees,urban residents,and primary hospitals.There are 14 influencing factors including hospital,gender,other payment methods,self-finance,spouse,and surgery.The analysis of the structural equation model shows that the length of hospital stay,hospital level,drug fees,treatment fees and age have a greater effect on hospitalization costs.The length of hospitalization not only has a direct effect on hospitalization costs,but also has an indirect effect.Conclusion: Treatment fees,drug fees,and inspection fees are the most important components of the patient’s hospitalization costs,but the service charges are relatively low,and the internal structure of hospitalization costs is unreasonable.The treatment fees and drug costs should be controlled to increase service costs.The component proportion is the key point to control the excessive growth of expenses;through path analysis and structural equation model,the main influencing factors of hospitalization expenses,such as the length of hospitalization,hospital grade,and the presence or absence of complications,should be taken from the main influencing factors,and more Targeted measures are taken to control costs.It is recommended to focus on controlling the growth of drug costs,appropriately reducing the impact of inspection items on costs,strictly standardizing clinical pathways,reasonably increasing the proportion of service item costs,and further reflecting the labor value of medical staff,and building diabetes The refined management system enriches the connotation of health management. |
