Objective: To evaluate the degree of steatosis in patients with MAFLD by ultrasonic attenuation imaging,shear wave elastography and shear wave dispersion imaging,and to explore the clinical value of ultrasound in quantitative evaluation of hepatic steatosis.Methods: 90 MAFLD patients with fatty liver diagnosed by transabdominal MRI in Hebei general Hospital from September 2020 to December 2020 were selected as MAFLD group,and 40 patients without fatty liver diagnosed by transabdominal MRI examination in the same period were selected as control group.The patients in both groups completed abdominal ultrasound examination within one week,and the general data of the patients were recorded.Ultrasonic measurements included ATI,SWE and SWD.In this study,the results of MRI-PDFF were taken as the reference standard to determine the correlation between the values of ultrasonic ATI,SWE,SWD and MRI,and the diagnostic efficiency of ultrasonic ATI in fatty liver was obtained by using the working characteristic curve of the subjects.Results:1.There were significant differences in BMI,ALT,AST,TG,GLU,ATI,SWE and SWD between the MAFLD group and the normal group,with significant difference(P<0.05);2.There were significant differences in waist circumference,BMI,ALT,ATI,SWE and SWD in different degrees of MAFLD group,with significant difference(P<0.05);3.The values of ATI,SWE and SWD measured by ultrasound were correlated with those of MRI-PDFF(the correlation coefficients were 0.825,0.697 and 0.576,respectively,P < 0.001);4.Ultrasonic ATI technique has good sensitivity and specificity in the diagnosis of fatty liver;5.The areas under the ROC curve of grade 1,grade 2 and grade 3 of hepatic steatosis diagnosed by ultrasonic ATI were 0.89,0.91 and 0.93,respectively,and the optimal thresholds for diagnosis were 0.63 d B/cm/MHz,0.72 d B/cm/MHz and 0.84 d B/cm/MHz;6.There was a significant correlation between ultrasonic ATI and the levels of metabolic state BMI,GLU,TG and CHOL with significant difference(P<0.05).Conclusion: ultrasonic ATI,SWE and SWD can be used to diagnose fatty liver quantitatively and qualitatively,in which ultrasonic ATI has the highest correlation with MRI,and ultrasonic ATI can grade the severity of fatty liver and is a reliable tool for quantitative detection of hepatic steatosis.
