| Background:In recent years,the incidence and prevalence of type 2 diabetes showed a significant trend of increase in many countries around the world.Currently,type 2 diabetes has become one of the most common endocrine system diseases in community general clinics and it has been included in the management of chronic non-communicable diseases in communities.T2 DM is a chronic lifelong disease with high disability rates and mortality which has become one of the major diseases that endanger the health of community residents.Since most patients have no obvious symptoms and signs at the early stage of the disease,the low awareness rates leads to the delay of the diagnosis of diabetes,which brings difficulties to treatment and great pressure to medical care providers.China is now in the stage of accelerated aging.With the aging of middle-aged and elderly people and the decline of physiological function,the incidence of T2 DM is increasing each year.However,the highly specialized modern medicine can not effectively solve the comprehensive problems of the elderly.How to improve the living quality of the elderly chronic patients in communities has become a common focus of the public and the medical community.At the present stage,China’s community health service system is inadequate,the government investment funds are insufficient,medical professionals are scarce,unable to meet the health needs of community residents.Guiding diagnosis and treatment by experts from three-level medical institutions in community health service institutions can optimize the limited medical resources,relieve the pressure of seeking medical treatment in urban general hospitals,and improve the clinical diagnosis and treatment in community health service institutions.Therefore,it is very beneficial to explore whether providing professional treatment opinions and consulting services to patients in community hospitals by experts from Grade A Class III Hospitals can effectively improve the blood glucose compliance rate and implement individualized treatment for T2 DM patients.It can also provide new ideas to solve the current problems of low blood glucose compliance rate of T2 DM patients and forming a long-term mechanism of expert community consultation in community.Objective:To explore whether the diagnosis and treatment mode of expert community consultation carried out by local community health service institutions can effectively improve the blood glucose compliance rate of T2 DM patients in the community,so as to provide relevant theoretical basis for the working mechanism of expert community consultation.Methods:From January 2017 to January 2018,150 cases of T2 DM patients who visited Taian City provincial health hospital were randomly selected as the research objects.Using the random number table method,the patients were divided into the observation group(visiting in the expert outpatient group)and the control group(visiting in the community general outpatient group),each with 75 cases.There was no significant difference between the two groups in gender,age and disease course(P>0.05).Observe the attainment of Hb A1 c,FPG,2h PG,BMI,TC,LDL-C and HDL-C in the two groups in six mouths.Results:1.The general data of gender,age,HbA1c,FPG,2h PG,BMI,TC,LDL-C,HDL-C in the two groups were comparable,and the difference was not statistically significant(P>0.05).2.After treatment,FPG,2h PG,Hb A1 c and BMI of the two groups were all significantly lower than before(P<0.05),and the decrease of FPG,2h PG and Hb A1 c was more obvious in the experimental group with statistically significant differences(P<0.05).TC in observation group and control group showed no statistically significant change after treatment(P>0.05).LDL-C decreased after treatment in the observation group(P<0.05),while no significant change was observed in the control group(P>0.05).Conclusions:1.To some extent,the local community T2 DM patients’ blood glucose control rate has been improved by the professional treatment opinions and consultation services provided by experts.Such as the blood glucose control rate of FPG,2h PG and Hb A1 c after treatment under the guidance of experts is significantly closer to the normal level than that in the community general outpatient group.2.According to the analysis of LDL-C,HDL-C and BMI,it was concluded that expert community consultation can promote T2 DM patients in long-term control of blood glucose compliance rate and improvement of blood lipid and metabolic level.3.Expert community consultation shows advantages in the control of blood glucose and lipid metabolism.Meanwhile,it also requires the joint participation of medical staff of relevant government departments and medical institutions at all levels as well as patients themselves,so as to realize the rational allocation of limited medical and health resources. |
