Objective: To explore the clinical application value of two-dimensional speckle tracking technology in evaluating left ventricular systolic function in patients with arteriovenous fistula.Methods: Thirty patients with arteriovenous fistula(all of which were caused by hypertensive nephropathy),30 patients with hypertension and 32 healthy patients were selected.After measuring the parameters of conventional echocardiography,the two-dimensional speckle tracking technology was used to measure the longitudinal strain and the circumferential strain of the subjects in each group,and the numerical differences between the groups were compared.Results:1.The peak longitudinal strain and the overall peak longitudinal strain on the two-chamber heart,three-chamber heart,and four-chamber heart views of the arteriovenous fistula group<hypertension group(P<0.05);two-chamber heart,three-chamber heart,and four-chamber heart views of the arteriovenous fistula group peak longitudinal strain and overall peak longitudinal strain <healthy group(P<0.05),the difference is statistically significant.2.The difference in peak circumferential strain and overall peak circumferential strain of the short-axis view of the apex,the short-axis view of middle portion,and the short-axis view of the basal portion in the arteriovenous fistula group,hypertension group,and the healthy group were not statistically significant(P>0.05).3.The circumferential strain in the same group of arteriovenous fistula group,hypertension group or healthy group were statistically significant,the apical short-axis view group>the middle short-axis view group>basal short-axis view group(P<0.05).4.The differences in the peak longitudinal strain and the overall peak longitudinal strain of the two-chamber heart,three-chamber heart,and four-chamber heart views in the same group of arteriovenous fistula group,hypertension group or healthy group were not statistically significant(P>0.05).Conclusion:The two-dimensional speckle tracking technology can quantitatively and accurately assess the changes in left ventricular local and overall myocardial contractile function in patients with arteriovenous fistula.Among them,the longitudinal strain is more sensitive.This technology can prompt early heart failure and provide an effective means for evaluating the treatment effect of patients with arteriovenous fistula.
