| Objective:To observe the therapeutic effect on the treatment of septic shock by dexmedetomidine combined with norepinephrine.Methods:38 patients with septic shock were randomly divided into control group and experimental group,with 19 cases in each group.Both in control and experimental groups were given early goal directed therapy.Those in control group we remaintained with norepinephrine,while those in experimental group were maintained with dexmedetomidine combined with norepinephrine.The initial dose of dexmedetomidine was in 1.0ug/kg/h.The length of mechanical ventilation and the length of stay in ICU were recorded as primary.The hemodynamic parameters including heart rate(HR)、mean arterial pressure(MAP)、central venous pressure(CVP)、cardiac index(CI)、extravascular lung water index(EVLWI)、systemic vascular resistance index(SVRI)were recorded after 6h、12h、24h、48h、72h treatment,as well as metabolism parameters including creatinine(Cr)、B-type natriuretic peptide(BNP)and troponin.Results:After treatment by dexmedetomidine combined with norepinephrine,The length of mechanical ventilation was shortened(18.42±2.59 days VS12.35±1.62 days,P<0.05)and the length of stay in ICU was also shortened(22.05±3.26 days VS14.06±2.93 days,P<0.05).HR value and Cr value in experimental group were decreased,while CI and SVRI value were increased.All above dates have significant differences(P<0.05).Conclusion:The results demonstrate that treating by dexmedetomidine combined with norepinephrine to septic shock have shown the positive effect on the septic patients by reducing patient heart rate(HR),enhancing heart function,improving blood pressure and contractility and improving kidney function,as well as by shortening the length of mechanical ventilation and the length of stay in ICU. |
