Objective:1.To evaluate the clinical effect and effective treatment course of Huayutongluo moxibustion on SIVD patients by observing the changes of MMSE,HDS-R and Mo CA at different time points before and after the intervention of Huayutongluo moxibustion on SIVD patients.2.Observe the differences in the white matter ultrastructure between SIVD patients and healthy people by diffusion tensor imaging(DTI)and fiber bundle tracking spatial statistical analysis(TBSS).3.To explore the effect of Huayutongluo moxibustion on the cerebral white matter ultrastructure by comparing the changes in the white matter ultrastructure of SIVD patients before and after the intervention.4.By comparing the differences of white matter fiber bundles between SIVD patients and healthy people,and combining with neuropsychological evaluation,the correlation between the ultrastructural changes of white matter and cognitive dysfunction was discussed.Methods:According to include and exclude standard screening 15 SIVD patients as treatment group,20 cases of healthy people as control group.Using Huayutongluo moxibustion intervene SIVD patients,before intervention,intervention after 12 weeks and 24 weeks,then respectively neuropsychological scale assessment and collection and brain magnetic resonance imaging(conventional and DTI sequence),and the control group after included to collect all data at a time;Neuropsychological score results were compared before and after SIVD the intervention,and imaging results were compared between SIVD patients and healthy control group using TBSS,and compared between SIVD patients with the intervention before and after.Results:1.Neuropsychological score results:MMSE: the difference after 12 weeks of treatment was statistical significance(P<0.05)compared with that before treatment,and the difference after 24 weeks was significant statistical significance(P<0.01).HDS-R: there was no statistical significance difference after 12 weeks of treatment(P>0.05)compared with that before treatment,while there was statistical significance difference after 24 weeks(P<0.05).Mo CA: there was no statistical significance difference after 12 weeks of treatment(P>0.05)compared with that before treatment,while there was statistical significance difference after 24 weeks(P<0.05).2.TBSS analysis results by DTI technology:2.1 Comparison between SIVD group before treatment and healthy control group:extensive white matter fiber bundle changes were observed in SIVD patients.The areas where FA value significantly lower are mainly distributed in: corpus callosum(mainly in the knee and a body),bilateral cingulate,on the left side of the beam under the longitudinal fasciculus,bilateral frontal pillow beam,thalamus after radiation,hook,double beam lateral prefrontal subcortical white matter,such as overall on the left side of the white matter fiber tracts change significantly more than the right side.(statistical threshold P<0.05,FWE correction)The areas where MD value significantly increased are mainly distributed in: corpus callosum(mainly in the knee and a body),fornix junction,left inferior fronto-occipital bundle,and left superior longitudinal bundle,such as overall on the left side of the white matter fiber tracts change more than the right side.(statistical threshold P<0.05,FWE correction)2.2 After 12 and 24 weeks of intervention,FA and MD values in the treatment group showed no significant difference.(statistical threshold P<0.05,FWE correction)Conclusion:1.Huayutongluo moxibustion is an effective method for clinical treatment of SIVD,which can improve the cognitive dysfunction of SIVD patients and effectively control the further progress of the disease.2.DTI technology can reflect the changes in the cerebral white matter ultrastructure in SIVD patients and can be used as the objective basis for the early diagnosis of SIVD.3.The damage of white matter fiber bundles in different brain regions of SIVD patients was correlated with cognitive dysfunction.4.Treatment for SIVD is a long-term process and may require a longer clinical course.
