Hepatocellular carcinoma is a high incidence malignant tumor in China.it has poor prognosis and short survival time.it is a major disease that seriously endangers the health of the people.Radiotherapy has become an important part of comprehensive treatment of HCC.The key to accurate radiotherapy for HCC lies in the determination of tumor target and the accurate prediction of radiation liver injury.In this study,we based on CT radiomics to finish the study of dynamic CT radiomic features assisted HCC accurate diagnosis and the variation of normal liver CT radiomic features with radiotherapy dose.In the study of dynamic CT radiomic features assisted HCC accurate diagnosis,the purpose was to assist the accurate diagnosis of HCC based on CT radiomics which can be used to distinguish normal liver tissue from primary hepatocellular carcinoma(HCC).Study on accurate diagnosis of HCC,38 HCC patients who underwent different contrast-enhanced CT scans including plain scan CT,arterial phase,portal venous phase,venous phase,and delayed phase contrast-enhanced CT,were enrolled in this study retrospectively.And 55 radiomic features were extracted from each gross target volume(GTV)of different phases.The ability to distinguish normal liver tissue from tumor target according to ROC curve.We found that there were 6 common features which the differences between normal liver tissue and GTV in arterial phase were the greatest in the comparison among five phases.4(Global Min,1Percentile,10 Percentile,Sum Variance)of 6 radiomic features which was statistically differences significant between normal liver tissue and GTV showed that the AUC were greater than 0.7,the Jordan value greater than 0.5,and the corresponding sensitivity greater than 0.85,the specificity were all greater than 0.7 except for 0.567-0.7 in the scanning period.And the AUC value was greater than 0.97 in the comparison between normal liver tissue and GTV at arterial phase.The sensitivity and specificity in arterial phase were significantly greater than that of other phases.The results indicated that CT radiomic features had high sensitivity and specificity in the accurate diagnosis of HCC,which could provide a new method for diagnosing HCC.In the study on the changes of CT imaging characteristics of normal liver with radiotherapy dose,the purpose was to find the Radiomics features which are closely related to the dose change,which can be used to detect and prevent the occurrence of RILD in the early stage.In the study of the correlation between characteristic changes and radiotherapy dose,56 patients who received abdominal radiotherapy with planning CT images and replanning CT images were research in this study retrospectively.The normal liver tissue was delineated on the planning CT image among the 5 dose ranges including 0-10 Gy,10-20 Gy,20-30 Gy,30-40 Gy,40Gy considered as region of interest(ROI)and then mapped to the replanning CT images by registration.Radiomic features were extracted from ROIs.Comparing the differences between these feature parameters in planning CT images and replanning CT images.We found that 828 features were extracted from each ROI.There were 390 features statistically significant differences among different parts of normal liver tissue before radiotherapy and between normal liver tissue before and after radiotherapy.There were statistically significant differences between the planning CT and replanning CT images in the range of 0-10 Gy,10-20 Gy,20-30 Gy,30-40 Gy,40Gy and above,respectively,25,23,21,58,41,respectively.The unique features were 13,12,4,28,and 24 respectively.The feature of Gray Level Variance changed significantly between planning CT images and replanning CT images in the doses range of 10-20 Gy,20-30 Gy,30-40 Gy.And 9 features changed significantly after receiving 20 Gy doses between planning CT images and replanning CT images.The wavelet transformed feature Gray Level Non Uniformity changed significantly after 10 Gy and above doses between planning CT images and replanning CT images.The results indicated that partial CT radiomic features had significant differences between planning CT and replanning CT images at different doses.Tracking these indicators was beneficial to analyze the changes of liver tissue with radiation dose,which was helpful for early detection and prevention of radiation-induced liver injury.Conclusion:Based on the radiomic features difference between HCC and normal liver tissue,it can assist the accurate diagnosis of liver cancer and the identification of tumor target area.The dynamic change of normal liver tissue with radiation dose can be found by CT radiomics which can be used for early prediction and diagnosis of radiation liver injury.
