Objective:Based on the mobile almond doctor app mobile network platform,this study developed and implemented a continuing care intervention plan,compared the pain scores,quality of life,and incidence of adverse drug reactions in patients with IDDS implanted home cancer pain before and after intervention to explore More measures and suggestions for the care of IDDS patients with cancer pain at home,improve the level of community care for patients with refractory cancer pain at home,provide a theoretical basis for the next step to build a continuous care model suitable for home cancer patients in China,and standardize the implantation of IDDS cancer pain The continuation of nursing routines for patients provides a scientific basis to comprehensively and effectively improve the quality of life of cancer patients.Methods:This study used a time-continuity study design of its own front-to-back controlled experiment,used a convenient sampling method,and selected 88 patients with cancer pain at home after implanting IDDS in Weihai City Hospital from January to June 2019 according to the inclusion and exclusion criteria,Continued nursing intervention based on the Almond Doctor app for patients who meet the inclusion criteria for 8 weeks,including pain score,quality of life score,and opioid adverse reactions before intervention,4 weeks during intervention,and 8 weeks after intervention Rate-related data were collected,repeated measurement analysis of variance was performed on the data,and the effect of intervention was evaluated.All data were analyzed using spssl 7.0 statistical software,measurement data were analyzed by t test and ANOVA single factor analysis,and count data were analyzed by X2 test,P<0.05 was considered statistically significant.Results:(1)General situation of the research objectThere were 88 patients with advanced cancer pain,aged 37-72 years old,all Han patients,including 51 males and 37 females.The education level constitutes 19 people with junior high school or below,43 people with high school or technical secondary school education,and 26 people with university education or above.All patients with marital status are married,and those who are divorced or widowed are classified as unparted,accounting for 15.92%,and the rest are partners with 84.08%.Households with a per capita monthly income of less than 3,000 yuan accounted for 21.59%,3,000 to 5,000 yuan accounted for 38.64%,and 5,000 yuan or more accounted for the largest proportion,39.77%.The medical insurance payment method constitutes 75%of employee medical insurance,18.18%of urban residents medical insurance,and the smallest proportion of patients paying at their own expense,which is 6.82%.(2)Comparison of pain scores of patients with cancer pain after implantation of IDDS before and after intervention:The pain score of patients before continuing nursing intervention was 4.05±1.45 points,the pain intensity was moderate to severe,and general demographic characteristics did not affect it.After 4 weeks of intervention,3.0±1.06 points,and after 8 weeks of intervention,the pain score was 1.05±0.76 points.The patients’ pain score gradually decreased with the extension of the intervention time,and the difference was statistically significant(P<0.05).(3)Comparison of patients’ quality of life before and after intervention:the total score of patients’ quality of life before continuing nursing intervention was(39.49±7.14)points,the quality of life satisfaction and total quality of life of those with partners were higher than those without partners,and the remaining general demographic characteristics It has no effect on all dimensions of quality of life.The total quality of life score was(50.89±5.59)points after 4 weeks of intervention,and the total quality of life score was(62.28±7.30)points after 8 weeks of intervention.The quality of life of patients gradually increased with the extension of intervention time,and the difference was statistically significant(P<0.05).(4)Comparison of incidence of opioid adverse reactions before and after intervention:the incidence of constipation before intervention was the highest,accoun-ting for 60.23%,followed by nausea and vomiting,the proportion was 46.59%,skin itching and urinary retention,accounting for 15.91%,respectively And 13.64%,the general demographic characteristics did not affect it.After 4 weeks of intervention,the incidence of constipation decreased by 15.91%,the incidence of nausea and vomiting decreased by 12.5%,the incidence of skin itching decreased by 4.55%,and the incidence of urinary retention decreased by 6.82%;after 8 weeks of intervention,the occurrence of constipation,nausea and vomiting,skin itching,and urine retention The rates were 48.87%,39.77%,1 3.64%,and 12.5%lower than before the intervention.Conclusion:(1)The pain intensity of patients with cancer pain implanted in IDDS before the intervention is moderate to severe,the scores of quality of life are low in all dimensions,and the incidence of opioid adverse reactions is lower than that of other cancer patients;(2)The intervention program based on the Almond Doctor app can better continue nursing intervention for patients with IDDS implanted home cancer pain;(3)Continued nursing intervention based on the Almond Doctor app can significantly reduce the incidence of pain scores,opioid adverse reactions and improve the quality of life of patients with IDDS implanted home cancer pain.
