| Objective:To explore the correlation between preoperative coagulation related parameters and endometrial cancer(EC),and to provide a reference for the early clinical diagnosis of EC.Methods:In this study,a total of 177 endometrial cancer patients(malignant group)from 45 to 75 years old who underwent surgical treatment in the gynecol ogical inpatient department of the affiliated hospital of Yanbian university from January 1,2011 to September 30,2019.173 patients with endometrial polyps(benig n group)diagnosed in the gynecology department of our hospital at the same ag e were collected.In addition,91 healthy women of the same age(normal group)were collected from the medical examinationdepartment of our hospital for heal thexamination.Record the general information of each group:thrombin time(TT),D-dimer(D-D),fibrinogen(FIB),prothrombin time(PT),activated partial thromboplastin time(APTT),hemoglobin(HB),lymphocyte count(LYMPH),lymphocyte percentage(L YMPH%),monocyte count(MONO),platelet count(PLT),total protein(TP),albumin(A LB),serum calcium(CA),the area under the curve(AUC)of the preoperative coagul ation parameters of each group was measuredby ROC curve,and the diagnostic v alue of each coagulation parameter in each group for EC was compared and an alyzed.Results:1.The three coagulation parameters-PT(P<0.001),FIB(P<0.001),APTT(P<0.05)in the malignant group were lower than the benign group;the body mass index(BMI)in the malignant group was higher than the benign group(P<0.001);the LYMPH%in the malignant group is lower than the benign group(P<0.001);2.The optimal cut-off value of fibrinogen(FIB)in diagnosis of endometrial cancer is 8.35,with a sensitivity of 70.2%,a specificity of 55.6%and an AUC of 0.658(95%CI 0.593-0.722)(P<0.001);The significance of FIB is 0.000 and the odds ratio is 0.713,suggesting that the difference is extremely significant.3.The optimal cut-off value of prothrombin time(PT)in diagnosis of endometrial cancer is 10.95,with a sensitivity of 75.4%,a specificity of 43.2%,and an AUC of 0.586(95%Cl 0.519-0.653)(P=0.015);The significance of PT is 0.321 and the odds ratio is 1.025,suggesting that the difference is extremely significant.4.The AUC of the activated partial thromboplastin time(APTT)is 0.527(95%CI 0.456-0.597)(P=0.452);The significance of APTT is 0.712 and the odds ratio is 0.991,suggesting that there is no significant difference.5.The significance of the combined analysis of the three coagulation parameters is 0.001(<0.01),the odds ratio is 25.872,suggesting that there is significant difference.6.PT is a risk factor for endometrial cancer(OR>1),while FIB and APTT are protective factors for endometrial cancer(OR<1).7.The BMI of the malignant group was greater than that in the normal group(P<0.05);the LYMPH%and LYMPH,CA,prognostic nutrition index(PNI),TP and ALB were all lower than those in the normal group(P<0.01).8.The ALB、TP、CA、PNI、LYMPH%and LYMPH in the benign group were all lower than those in the normal group(P<0.01).9.There was no significant difference in LYMPH、TP、ALB、CA、PNI、HB、D-D between malignant group and benign group.Conclusions:1.FIB and PT have certain clinical significance for the diagnosis of endometrial cancer,while APTT has no obvious diagnostic significance.2.The combined analysis of various coagulation parameters has certain clinical significance for the diagnosis of endometrial cancer. |
