| Objective:To analyze the factors affecting the success rate of superficial lymph nodes core needle biopsy under ultrasound guidance,and to summarize the techniques of puncture in superficial lymph nodes.Methods:The clinical data and Pathological diagnoses of 188 patients who underwent ultrasound-guided superficial lymph nodes core needle biopsy in the First Affiliated Hospital of Guangxi Medical University from January 2017 to December 2019 were collected.The influence of patient factors(age,gender),lymph node factors(long diameter,short diameter,puncture site,presence or absence of necrosis,depth)and operative factors(type of puncture needle,number of puncture)on the puncture success rate were analyzed.Results:(1)159 cases of 188 patients were diagnosed by biopsy,and the success rate of biopsy was 84.6%(159/188),Among them,there were 115 cases of malignant lesions,accounting for 72.3%(115/159),and 40 cases of benign lesions,accounting for 25.2%(40/159),4 cases of normal lymph nodes,accounting for 2.5%(4/159).(2)The results of this study showed that the long diameter of lymph nodes(t=-2.069,P=0.040),the presence or absence of necrosis(c~2=5.001,P=0.025)were the independent influencing factors for the success rate of ultrasound-guided biopsy of superficial lymph nodes aspiration.The larger the long diameter of lymph nodes,the higher the success rate of puncture(OR=1.488,P=0.036).The higher the internal necrosis rate,the lower the puncture success rate(OR=0.304,P=0.032).The number of punctures in the successful group was less than that in the failed group(t=2.041,P=0.043).Age,gender,short diameter,puncture site,depth and type of needle had no significant influence on the puncture success rate(P>0.05).Conclusion:(1)The long diameter of lymph nodes and the presence or absence of necrosis all affect the success rate of superficial lymph node biopsy,(2)The larger the long diameter of the lymph nodes,the higher the success rate of puncture.Therefore,mastering the puncture skills of small lymph nodes can improve the success rate of puncture;the absence of necrosis inside the lymph nodes,the higher the success rate of puncture.,avoiding the necrotic area by multi-mode ultrasound can effectively improve the success rate of puncture.(3)The number of punctures in the successful group was less than that in the failed group.In the puncture,if the sampling was good,the number of puncture would not be increased,Puncture twice is best;If the sampling is insufficient,the number of punctures should be increased or other biopsy methods should be adopted according to the situation. |
