| Objective: To evaluate the diagnostic or monitoring value of neurofilament light chain in the serum / plasma by systematic evaluation.Method: Searching Pubmed,Embase,Cochrane Library,Web of Science on the internet,searching term “neurofilament light chain proteins,NfL,Alzheimer’s disease,AD,frontomenporal dementia,FTD,mild cognitive impairment,MCI,Parkinson’s disease,PD,Parkinson’s disease dementia,PDD,amyotrophic lateral sclerosis,ALS,corticobasal syndrome,CBS,multiple system atrophy,MSA,progressive supranuclear palsy,PSP,dementia with Lewy bodies,DLB,serum,plasma” The time range for inclusion was January 2000 to December 2019.The search language was restricted to English,and the original documents and related meta-analyzed references were expanded.Bayesian-based documents were used for documents that met the inclusion criteria.Stata’s meshed meta-processing software Stata 13.0 performs statistics and analysis of data.Results: A total of 11 studies were included in this article to evaluate the serum /plasma NfL expression levels of neurodegenerative diseases,a total of 2999 patients,of which 1282 were healthy controls and 1,717 were neurodegenerative diseases(AD n=505;FTD n=87;MCI n=908;PD n=197;PDD n=23).Compared with healthy controls,patients with FTD,AD,MCI,and PD had higher serum / plasma NfL expression levels(SMD and 95% CI were greater than 0,and did not include 0),with statistical differences,suggested that the increased expression of serum / plasma NFL is significantly related to neurodegenerative diseases.The ranking of diagnostic value of serum / plasma NFL in neurodegenerative diseases suggests that FTD is the highest(Sucra = 98.3%,Mean Rank = 1.1),ad is the second(Sucra = 66.7%,Mean Rank =2.7),PD is the third(Sucra = 53.9%,Mean Rank = 3.3),PDD is the fourth(Sucra =44.9%,Mean Rank = 3.8),MCI is the fifth(SUCRA=32.5%,Mean Rank=4.4),HC is the lowest(Sucra = 3.7%,Mean Rank = 5.8).Conclusion: The level of serum / plasma NFL expression in patients with AD,FTD,MCI and PD was significantly higher than that in healthy controls.From the point of view of Sucra sequencing,the serum / plasma NFL expression level may have the highest diagnostic value for FTD.The expression level of serum / plasma NFL is significantly related to neurodegenerative diseases,which is expected to become an influential indicator in neurodegenerative diseases.It can be used as a good alternative measure to check CSF NFL,help clinicians to make early diagnosis of neurodegenerative diseases,so that patients can get timely intervention. |
