Objective To assess the effects of WeChat platform-based comprehensive cognitive intervention on the cognition and changes of serum homocysteine(HCY),uric acid(UA),low density lipoprotein(LDL),triglyceride(TG)and total cholesterol(TC)levels in patients with post-stroke cognitive impairment no dementia(PSCIND).The feasibility of WeChat platform in the management of PSCIND was also explored.Methods The cognitive function assessment was performed in patients with ischemic stroke at the Department of Neurology,Second Affiliated Hospital of Hainan Medical College from June 2018 to September 2018.65 patients who meet the inclusion criteria,are divided into two groups randomly by numerical random table method:33 cases in the control group and 32 cases in the experimental group.Patients in the control group received usual medications.In addition to usual care,those in the experimental group also received health education,cognitive training,remote family rehabilitation guidance and psychological counseling through the WeChat management platform,and the subjects were regularly supervised to help them complete the training on time.Pre-intervention and 3 months and 6 months after the intervention,the Mini Mental State Examination Scale(MMSE)and the Montreal Cognitive Assessment Scale(MoCA)were used to perform cognitive assessment.Serum HCY,UA,LDL,TG and TC levels were assessed at pre-intervention and 6-month after the intervention.Results1.During the intervention,3 cases fell off in the control group and 1 case fell off,1 case died in the experimental group.Finally,30 cases in each group were taken into statistics.At baseline,there was no significant difference in the MMSE scale and MoCA scale between two groups(P>0.05).The MMSE and MoCA scores significantly increased after 3-or 6-month intervention compared with pre-intervention(P<0.05),and both scores were significantly higher in the experimental group than those in the control group(P<0.05).2.At the 3 months post-intervention,there was no significant difference in most items of MoCA scores(P>0.05)except for visual space and executive function.At 6 months post-intervention,the scores of most items(P<0.05)except for abstract memory were significantly higher in the experimental group.3.There was no significant difference in HCY,UA,LDL,TG and TC levels between two groups at baseline(P>0.05);All these parameters decreased significantly in both groups after 6-month intervention compared with baseline(P<0.05),and HCY,LDL,TG and TC levels in the experimental group were significant lower than those in the control group at the 6 months post-intervention(P>0.05).Conclusions1.WeChat platform-based comprehensive intervention involving health education,cognitive training,remote family rehabilitation guidance and psychological counseling could improve post-stroke cognitive function in patients with PSCIND patients.2.Serum HCY,LDL,TG and TC were significantly improved by WeChat platform-based comprehensive intervention,which may be associated cognitive function improvement.The mechanism needs to be further explored.3.WeChat platform-based comprehensive intervention was demonstrated to contribute to the improvement of cognitive function,which deserved to be disseminated in clinical practices.
