Clinical And Experimental Study Of Shengmaiyin Combined With Atorvastatin In The Treatment Of Diabetic Cardiomyopathy

Objective:1.To observe the clinical effect of Shengmaiyin combined with atorvastatin on Qi-yin deficiency syndrome of diabetic cardiomyopathy.2.To explore the protective effect of Shengmaiyin combined with atorvastatin on the myocardium of diabetic cardiomyopathy rats.Methods:1.Clinical research:60 patients with diabetic cardiomyopathy with Qi-Yin deficiency syndrome were selected and divided into two groups according to the random number table method,with 30 cases in the control group and the other in the treatment group.Both groups were given conventional symptomatic treatment,while the control group was given atorvastatin calcium(lipitor)20mg/d on the basis of conventional treatment.The treatment group was given Shengmaiyin orally on the basis of the treatment of the control group,the clinical efficacy and changes in TCM syndromes,HbA1C,LDL-C,CRP,LVEF and E/A were observed after 1 month.2.Experimental study:Forty rats were randomly divided into a blank control group(group A,n=10)and a treatment group(n=30).The treatment group was given a high-sugar and high-fat diet for 4 weeks,and STZ(40mg/kg)was injected intraperitoneally to establish a diabetic cardiomyopathy rat model.After successful modeling,they were randomly divided into three groups(groups B,C,and D,Per group n=10).Group B was given metformin 200mg/kg for hypoglycemic therapy,and groups C and D were given atorvastatin 2.lmg/kg,atorvastatin 2.1mg/kg+Shengmaiyin 21ml/kg for 8 weeks.Fasting blood glucose was detected in rats,and HE staining,Masson staining,and TUNEL staining were used to observe myocardial injury,fibrosis,and apoptosis in rats.Results:1.The scores of TCM syndromes,the levels of LDL-C and CRP in the two groups of patients were both evidently ameliorated as compared with those before treatment(P<0.05),but there was no significant difference in LVEF and E/A(P>0.05).The scores of TCM syndrome,HbAlC,and LDL-C in the treatment group were improved in comparison with the control group(P<0.05).2.The myocardial collagen fiber fraction and apoptosis index of the C and D groups were significantly reduced(P<0.01),myocardial damage was relatively mild in comparison with group B.The level of blood glucose in group D was significantly improved,and the myocardial collagen fiber fraction and apoptosis index were relatively reduced in comparison with group C(P<0.01).Conclusion:The TCM syndromes and the levels of HbAlc,LDL-C and CRP in patients with diabetic cardiomyopathy with Qi-Yin deficiency syndrome can be effectively improved by Shengmaiyin combined with atorvastatin.Abnormal glucose metabolism,myocardial fibrosis and apoptosis,and myocardial injury in diabetic cardiomyopathy rats can also be effectively inhibited.
