Objective:To observe the correlation between H-type hypertension and Traditional Chinese Medicine(TCM)syndromes in middle-aged and elderly patients,and to explore H-type hypertension from the perspective of TCM,and to analyze the carotid plaque of different syndromes of H-type hypertension.Methods:A total of 211 elderly patients with H-type hypertension were selected.According to the uniform four diagnosis information collection questionnaire to carry on the questionnaire survey to the patient,on the premise of following the four diagnosis principles of traditi onal Chinese medicine,the included cases were observed and recorded in detail and accurately,collecting generally and carotid ultrasound was performed,and carotid IMT index and Crouse score were collected.The level of homocysteine(Hcy)was determined by cyclic enzymatic method.Results:(1)The TCM syndrome types involved were divided into four categories,which were successively distributed from more to less:phlegm-dampness stagnation(32.2%);Yin deficiency and Yang hyperactivity(28.9%);liver-fire hyperactivity(21.8%);Deficiency of Yin and Yang(17.1%);(2)Comparison of general conditions:Among 211 middle-aged and elderly patients with h-type hypertension,127 were male patients and 84 were female patients,with a male to female ratio of about 1.51:1.Among the four groups,the elderly patients with deficiency of Yin and Yang type were the oldest,and the difference between them and the other groups was statistically significant(P<0.05).There were no significant differences in gender,smoking,heart rate and accompanying medical history among the four groups(P>0.05)(3)Four groups of syndromes were statistically difference in blood pressure,phlegm-dampness stagnation and liver-fire hyperactivity type’s systolic and diastolic blood pressure levels were higher than the other two types(P<0.05);Yin and Yang deficiency with the longest duration,the course of hypertension of it compared with Yin deficiency type hand to compare difference was statistically significant(P<0.05)(4)The levels of Glu and HbAlc in patients with Yin deficiency and Yang hyperactivity were the highest,and the differences between the two groups were statistically significant(P<0.05).Patients with excessive phlegm-dampness had the highest levels of Hcy,Scr and BUN(P<0.05)(5)The carotid artery Crouse score of deficiency of Yin and Yang was the highest,there was significantly difference compared to Yin deficiency and Yang hyperactivity and liver fire hyperactivity types,and the difference was statistically significant(P<0.05)(6)Spearman correlation analysis showed that carotid ultrasound Crouse score was correlated with age,course of hypertension,SBP,pulse pressure difference,HR,Hey,HDL-C,HbAlc,Glu,Scr and DD2 levels(P<0.05)It was negatively correlated with HR and HDL-C,and positively correlated with age,course of hypertension,SBP,pulse pressure,Hey,HbAlc,Glu Scr and DD2.(7)Through comparison and synthesis,it can be seen that among the four syndrome types,the carotid atherosclerosis degree of patients with phlegm-dampness stagnation and deficiency of Yin and Yang syndrome is more significant.Conclus ion:(1)The most common TCM syndrome in elderly patients with H-type hypertension is phlegm-dampness stagnation syndrome.(2)The blood pressure levels of phlegm-dampness stagnation and hyperactivity of liver fire were the highest,the blood glucose level of H-type hypertension with Yin deficiency and Yang hyperactivity was the highest.(3)The syndromes of phlegm-dampness and deficiency of Yin and Yang are closely related to the carotid artery Crouse score in middle-aged and elderly patients with h-type hypertension.(4)The higher the Hey level in middle-aged and elderly patients with h-type hypertension,the higher the levels of blood pressure,blood sugar,blood lipid and carotid artery plaque,and smoking was associated with Hey level in male patients(5)Carotid ultrasound Crouse score was correlated with age,course of hypertension,smoking,complications(diabetes,coronary heart disease,cerebral infarction),Hey level,blood glucose.(Glu,HbAlc),and HDL-C,suggesting that clinical attention should be paid to the control of these risk factors to prevent and control the development of carotid atherosclerosis.(6)More attention should be paid to the phlegm-dampness stagnation and the deficiency of Yin and Yang syndrome in H-type hypertension,and active intervention should be carried out as early as possible to reduce or delay the occurrence and development of clinical events.
