| ObjectiveTo compare the application of ultrasound-guided transhumeral basilic vein puncture and central vein catheterization(CVC)in healthy donors during peripheral blood hematopoietic stem cell collection,in order to provide a theoretical basis for medical staff to choose appropriate and safety catheterization method.It may be useful to complete the peripheral blood hematopoietic stem cell collection standardly.Methods1.This research was a quasi-experimental study.From December 2016 to June2018,193 healthy hematopoietic stem cell donors in a grade A hospital in Zhengzhou were included in the study.From December 2016 to June 2017,82 healthy donors were recruited as the control group.CVC was used to collect peripheral blood hematopoietic stem cells.111 healthy donors from July 2017 to June 2018 were recruited as the intervention group.Peripheral blood stem cells were collected by ultrasound-guided transhumeral basilic vein puncture.2.The differences of collection flow velocity,collection low-pressure alarm times,the number of MNC and CD34 positive cells,success rate of one-time insertion,duration of catheterization,puncture satisfaction and the incidence of complications were observed between the two groups.3.The data was entered into R data entry software.Using frequency,and percentage to describe the enumeration data;using mean,standard deviation to describe the measurement data.If the normality and homogeneity of variance were met,two independent sample t-test was used;if one of the conditions was not met,the difference of collection flow velocity,low-pressure alarm times,the number of MNC and CD34 positive cells,and duration of catheterization between the two groups were compared by rank sum test;chi-square test/corrected chi-square test/Fisher exact probability method was used to analysis success rate of one-time insertion,puncture satisfaction and the incidence of complications between the two groups.All the tests showed significant difference with P<0.05.The level of alpha was 0.05.Results1.A total of 193 healthy donors met the requirements.111 health donors in the group of ultrasound-guided transhumeral basilic vein puncture,The average age was(30.82±14.98)year old;82 in the CVC group and that of the group was(31.96±16.36)year old.2.The average collection flow velocity of the ultrasound-guided transhumeral basilic vein puncture group was(57.54±9.54)ml/min,which was little higher than that of the CVC group((53.53±13.91)ml/min),however,the difference was not statistically significant(P>0.05).3.The results show that collection low-pressure alarm times in the CVC group was significantly larger than that in the ultrasound-guided transhumeral basilic vein puncture group,and there is a significant difference(P<0.05).4.The number of MNC and CD34 positive cells in the ultrasound-guided transhumeral basilic vein puncture group was(4.88±0.18)×10~6/kg and(4.76±0.45)×10~8/kg respectively,which were not statistically significant compared with(4.97±0.12)×10~6/kg and(5.15±0.59)×10~8/kg in the CVC group(P>0.05).5.The puncture satisfaction of the ultrasound-guided transhumeral basilic vein puncture group was 90.1%,which was higher than that of the CVC group(73.2%),and there was statistical difference between the two groups(P<0.05).success rate of one-time insertion of the basilic vein group was 60.4%,and that of the CVC group was 36.6%,there was big difference between the two groups(P<0.05).The duration of catheterization was(22.10±5.21)min,which was shorter than that of the CVC group((24.62±6.02)min).The difference between the two groups was not statistically significant(P>0.05).6.The incidence of complications in the ultrasound-guided transhumeral basilic vein puncture group and CVC group was 5.4%and 14.6%separately,which is statistically different(P<0.05).ConclusionDuring collecting peripheral blood hematopoietic stem cells,ultrasound-guided basilic vein puncture has the same collection efficiency and collection flow velocity compared with CVC.However,ultrasound-guided basilic vein puncture has less collection low-pressure alarm times,higher success rate of one-time insertion,higher puncture satisfaction and lower incidence of complications,which is worthy of promotion. |
