BackgroundThe world is entering an era of aging and longevity.Compared with the world,China’s elderly population has a large amount,and the aging process is fast.At the same time,health problems of the elderly in China are prominent.And health is a multi-dimensional concept.The health evaluation indicators of the elderly have been developing and updating.As a new health measurement indicator,cognitive frail is more and more popular.Age and gender are important factors of health,as well as an important reference for formulating health-related policies.Studying the relationship between cognitive frail,the new health indicator,and its relationship with age and gender is useful to understand the age and gender distribution differences of cognitive frail,and provide reference for related policies and research.ObjectiveThis study hopes to achieve the following goals:(1)Describe the cognitive level,frail and cognitive frail of the elderly in China;(2)Describe the age and gender differences of cognitive frail of the elderly;(3)Study the risk of cognitive frail of the elderly in different groups from the perspective of age and gender;(4)Analyze the risk of cognitive frail of the elderly in different gender and age groups.Summarize the differences of cognitive frail in different gender and age;(5)Based on the research conclusions above,combined with domestic and foreign literature,put forward promotion strategy to improve health status of the elder.MethodIn this study,we used the data of CLHLS 2018 database,selected people 65 years old or older as the research sample,and constructed the measurement index of cognitive frail through two indicators of Mini Mental State Examination(MMSE)and frailty index(FI).By means,standard deviation,median,frequency and so on to describe the cognitive function,frail and cognitive frail of the elderly;Analyze the age differences of cognitive frail;under the control of other factors,by using multiple linear regression to explore the influence of age and gender on cognitive level and frail index of the elderly,by using multi category logistic regression to study the risk of cognitive frail in different age and sex group.ResultsAs the results of descriptive study,a total of 15410 elderly people was included in this study,including 2525(16.39%)who were completely health,9386(60.91%)with mild cognitive impairment or frail,1891(12.27%)with mild cognitive frail,and 1608(10.43%)with serious cognitive frail.The average score of the cognitive level of the elderly was 7.38,and the cognitive function of women was worse than that of men;the average of the frail index was 0.259,and women were weaker than men.With the increase of age,the proportion of completely healthy elderly gradually decreases,while the proportion of serious cognitive frail elderly gradually increases.The proportion of the elderly with mild cognitive impairment or frail and the elderly with mild cognitive frail increased with age.The results of regression analysis show that compared with men,women have a higher risk of cognitive frail;compared with low-age people,high-age people have a higher risk of cognitive frail.In terms of gender,the risk of mild cognitive impairment or frail,mild cognitive impairment and serious cognitive frail in the elderly increased steadily with age compared with that in the low-income men.Taking the completely healthy people as the control group,the risk of serious cognitive frail of men in the 100 years old group was 100 times higher than that in the 75-79 years old group;compared with the low-income women,the risk of mild cognitive frail in the high-income women was higher.The risk of cognitive impairment or frail,mild cognitive frail and serious cognitive frail showed an inverted U-shaped pattern,which increased rapidly with age and then decreased slightly.Taking the completely healthy people as the control group,the risk of serious cognitive frail of women in 100 years old group was 300 times higher than that in 75-79 years old group.Conclusion,suggestion and ProspectThe results show that:(1)In general,the cognitive frail of the elderly is serious in China;(2)The age and gender differences of cognitive frail are obvious,the cognitive frail of the female elderly is more serious than that of the male,and the cognitive frail of the elderly is more serious than that of the younger elderly.The risk of cognitive frail in the elderly shows a trend of rapid increase with the increase of age;(3)the deteriorate speed of cognitive frail in different age and gender groups is different,so intervention measures should be taken differently.Suggestions and prospects:This study found that cognitive frail presents great age and gender differences.On the one hand,we should pay more attention to the health of the elderly and promote health equity;on the other hand,we should strengthen the research on cognitive frail and study the prevention and intervention measures of cognitive frail.
